摘要
目的分析超声造影结合弹性成像检查用于甲状腺微小乳头状癌诊断的效能。方法选取江苏省连云港市东海县人民医院2019年1月至2023年4月收治的疑似甲状腺微小乳头状癌患者78例,所有患者均开展超声造影及弹性成像检查,对比病理学诊断结果,分析超声造影、弹性成像及二者联合检查的诊断结果,同时分析二者及联合检查用于甲状腺癌微小乳头状癌诊断中的效能(敏感度、特异度、准确度),比较不同性质病灶的超声造影参数及弹性评分。结果超声造影联合弹性成像诊断甲状腺微小乳头状癌的敏感度、特异度、准确度、阳性及阴性预测值分别为94%、98%、96%、97%、95%,均高于超声造影、弹性成像诊断结果(P<0.05)。恶性组造影灌注峰值(Peak)、最大灌注强度(SImax)及平均灌注强度(SImean)均低于良性组,超声弹性评分高于良性组(P<0.05)。恶性组超声造影的增强强度、均匀增强、早期消退模式与良性组比较,差异无统计学意义(P>0.05)。结论超声造影结合弹性成像检查用于甲状腺微小乳头状癌诊断,能够提高恶性结节检出率,值得临床应用。
Objective To analyze the efficacy of contrast-enhanced ultrasound combined with elastography in the diagnosis of thyroid micropapillary carcinoma.Methods A total of 78 suspected patients with thyroid micropapillary carcinoma admitted to the Donghai People′s hospital from January 2019 to April 2023 were selected,and contrast-enhanced ultrasound and elastography were carried out on all of them to compare the pathological diagnostic results,to analyze the diagnostic results of ultrasonography,elastography and their combined examination were analyzed.At the same time,the efficacy(sensitivity,specificity,accuracy)of the two and the combined examination in the diagnosis of thyroid micropapillary carcinoma was analyzed,and the parameters of contrast ultrasound and elastic scores of different lesions were compared.Results The sensitivity,specificity,accuracy,positive and negative predictive values of CEUS combined with elastography in the diagnosis of thyroid micropapillary carcinoma were 94%,98%,96%,97%and 95%,respectively,which were higher than those of diagnosis by CEUS and elastography(P<0.05).The maximum value(PEAK),maximum perfusion intensity(SImax)and average perfusion intensity(SImean)of the malignant group were lower than those of the benign group,and the ultrasonic elasticity score was higher than that of the benign group(P<0.05).The enhancement intensity,even enhancement and early fading mode of CEUS in malignant group were compared with those in benign group,the difference was not statistically significant(P>0.05).Conclusion Enhanced ultrasonography combined with elastography in the diagnosis of thyroid micropapillary carcinoma improves the detection rate of malignant nodules and is worthy of clinical application.
