摘要
目的观察中药穴位贴敷联合中医饮食、情志调护对癌痛患者服用阿片类药物所致便秘伴焦虑的干预效果。方法将100例服用阿片类药物所致便秘伴焦虑的癌痛患者按照随机数字表法分为对照组(n=50例)和观察组(n=50例)。对照组患者给予常规治疗和中医饮食、情志调护等护理,观察组患者在对照组基础上给予中药穴位贴敷,连续干预1周,对比两组干预效果。结果观察组的便秘干预临床效果更好,有效率高达96.00%,显著高于对照组的82.00%(P<0.05);干预后,观察组的疼痛NRS评分显著低于对照组(P<0.05)。观察组的粪便性状Bristol评分、排便难度CCS评分均显著低于对照组(t=-13.876,-8.9240,P<0.05);与对照组相比,观察组患者的焦虑SAS评分更低,睡眠时间显著增加,PSQI评分显著降低(P<0.05);两组患者的PAC-QOL评分逐渐降低,治疗3d、干预7d同一时间点比较,均以观察组的生活质量PAC-QOL评分显著低于对照组(t=-20.407,-36.043,P<0.05);观察组的神经递质5-HT含量显著升高(P<0.05)。结论中药穴位贴敷联合中医饮食、情志调护治疗服用阿片类药物所致便秘伴焦虑的癌痛患者的效果理想,更利于减轻癌痛、改善排便、提高生活质量,其起效可能与影响血清中5-HT等神经递质含量变化有关,值得推广。
Objective To observe the intervention effect of Chinese medicine acupoint application combined with Chinese medicine diet and emotional conditioning on constipation and anxiety caused by opioid drugs in patients with cancer pain.Methods 100 patients with opioid-induced constipation and anxiety were divided into control group(n=50)and observation group(n=50)according to random number table method.Control group was giv⁃en conventional treatment and nursing,observation group was given Chinese medicine acupoint application com⁃bined with Chinese medicine diet,emotional conditioning on the basis of the control group,continuous inter⁃vention for 1 week and compared the two groups.Results The clinical effect of constipation intervention in the observation group was better,and the effective rate was as high as 96.00%,significantly higher than that of the control group 82.00%(P<0.05).After intervention,the pain NRS score in the observation group was significant⁃ly lower than that in the control group(P<0.05).Fecal trait Bristol score and defecation difficulty CCS score in observation group were significantly lower than those in control group(t=-13.876,-8.9240,P<0.05).Com⁃pared with the control group,observation group had lower anxiety SAS score,significantly increased sleep time,and significantly decreased PSQI score(P<0.05).The PAC-QOL score of 2 groups gradually decreased,and the PAC-QOL score of the observation group was significantly lower than that of the control group at the same time point after 3 days of treatment and 7 days of intervention(t=-20.407,-36.043,P<0.05).The content of 5-HT in the observation group was significantly increased(P<0.05).Conclusion Chinese medicine acupoint stick⁃ing combined traditional food,modern regulating therapy of opioid drugs caused by constipation patients with anxiety of pain with the effect of the ideal,more conducive to relieve pain and improve defecation,reduce anxi⁃ety,improve sleep quality,quality of life,the work may affect serum 5-HT neurotransmitter content
