摘要
目的探讨第三代头孢菌素类抗生素致患者凝血功能异常的危险因素。方法选择我院2020年7月至2023年1月应用第三代头孢菌素类抗生素治疗的患者206例,观察凝血功能异常发生情况,收集临床资料进行凝血功能异常的单因素分析,并将有统计学意义的因素作为自变量纳入Logistic回归分析。结果共有56例患者发生凝血功能异常,发生率27.18%;单因素分析显示,患者凝血功能异常发生率与年龄、肾小球过滤率、白蛋白水平、合并症、抗凝药使用情况有关(P<0.05)。Logistic回归分析显示年龄≥65岁、肾小球过滤率异常、白蛋白水平异常、有合并症、使用抗凝药均为第三代头孢类抗生素致患者发生凝血功能异常的危险因素(OR>1,P<0.05)。结论第三代头孢菌素类抗生素用于年龄大、肾功能异常、白蛋白水平异常、有合并症及使用抗凝药的患者更易发生凝血功能异常,需重视此类患者的凝血功能监测。
Objective To explore the risk factors of coagulation dysfunction in patients caused by third-generation cephalosporin antibiotics.Methods 206 patients admitted with third-generation cephalosporin antibiotics in our hospital from July 2020 to January 2023 were selected to observe the occurrence of coagulation dysfunction.Clinical data were collected for univariate analysis of coagulation dysfunction,and statistically significant factors were selected as independent variables in Logistic regression analysis.Results A total of 56 patients had abnormal coagulation function,with the incidence of 27.18%.Univariate analysis shows that the incidence of abnormal coagulation function is correlated with age,glomerular filtration rate,albumin levels,comorbidities and use of anticoagulants(P<0.05).Logistic regression analysis showed that≥65 years old,abnormal glomerular filtration rate,abnormal albumin levels,comorbidities,and use of anticoagulants were risk factors for patients with coagulation dysfunction caused by third-generation cephalosporins antibiotics(OR>1,P<0.05).Conclusions Third-generation cephalosporin antibiotics are more likely to cause coagulation abnormalities in patients with older age,renal dysfunction,abnormal albumin levels,comorbidities,and use of anticoagulants.It is necessary to pay attention to the monitoring of coagulation function in such patients.
