摘要
目的分析鲍曼不动杆菌(ABA)的临床分布及其耐药性趋势。方法选取2022年9月至2023年2月东莞市人民医院检验科微生物实验室接收的各科室送检的病原学标本103份,分析ABA来源情况、分布科室以及耐药情况。结果ABA标本主要来源于痰液、血液、创口分泌物、尿液等标本,以痰液标本(83.50%)所占比例最高,其次为分泌物标本(9.70%)、尿液标本(5.83%)、肺泡灌洗液标本(0.97%)。ABA感染科室中以呼吸与危重科(18.45%)所占比例最高,其次为急诊ICU(15.53%)、ICU(13.59%)、肿瘤科(4.85%)、肾内科(4.85%)。ABA对环丙沙星耐药率(90.29%)最高,头孢吡圬(79.61%)、头孢他啶(76.70%)、阿米卡星(71.84%)耐药率次之,对多粘菌素B(0%)、替加环素(0.97%)耐药性较低。结论ABA在临床检出率较高,对环丙沙星、头孢吡圬等抗菌药物存在一定的耐药性,临床应全面了解ABA分布特点及耐药性情况,根据药敏试验结果规范使用抗菌药物,尽可能降低耐药率。
Objective To analyze the clinical distribution and drug resistance trends of Acinetobacter baumannii(ABA).Methods From September 2022 to February 2023,103 pathogenic samples were selected from the microbiological laboratory of Department of Laboratory of Dongguan People's Hospital,and the source,distribution and drug resistance of ABA were analyzed.Results ABA specimens mainly came from sputum,blood,wound secretions,urine,and other specimens,with sputum specimens(83.50%)accounting for the highest proportion,followed by secretion specimens(9.70%),urine specimens(5.83%),and alveolar lavage fluid specimens(0.97%).Among ABA infection departments,respiratory and critical care departments(18.45%)account for the highest proportion,followed by emergency ICU(15.53%),ICU(13.59%),oncology department(4.85%),and nephrology department(4.85%).The drug resistance rate of ABA to Ciprofloxacin(90.29%)was the highest,followed by Cefepime(79.61%),Ceftazidime(76.70%),Amikacin(71.84%),and the drug resistance to polymyxin B(0%)and Tigecycline(0.97%).Conclusion The clinical detection rate of ABA is high,and there is a certain resistance to Ciprofloxacin,Cefepime and other antimicrobial agents.The distribution characteristics and drug resistance of ABA should be comprehensively understood in clinical practice,and the use of antimicrobial agents should be standardized according to the drug sensitivity test results,so as to reduce the drug resistance rate as much as possible.
