摘要
目的:观察痛风康(丸剂)联合秋水仙碱治疗痛风性关节炎的临床疗效。方法:按随机平行对照原则,将80例患者分为治疗组和对照组。两组均给予痛风性关节炎的基础治疗,药物治疗均口服秋水仙碱,治疗组在对照组治疗的基础上加用痛风康(黄柏、苍术、薏苡仁、川牛膝、土茯苓、虎杖、萆薢、徐长卿、三七、地龙、全蝎、络石藤等,制成丸剂),治疗1周后观察红细胞沉降率(Erythrocyte Sedimentation Rate,ESR)、C反应蛋白(C-reactive Protein,CRP)、血尿酸(Uric Acid,UA)、视觉模拟评分法(Visual Analog Scale,VAS)疼痛评分变化,停用秋水仙碱,治疗组继续口服痛风康,1个月后观察UA变化。结果:治疗1周后,治疗组的显效率为85.0%(34/40),高于对照组的72.5%(29/40),但两组的显效率比较差异无统计学意义(P> 0.05),治疗组的痊愈率为60.0%(24/40),明显高于对照组的37.5%(15/40)(P <0.05),差异有统计学意义。治疗1周后,治疗组的VAS疼痛评分、CRP及ESR水平均明显低于对照组(P <0.05)。两组患者的UA均有一定程度的下降,治疗1周后两组UA下降水平无明显差异(P=0.40),但治疗1个月后,治疗组的UA水平明显低于对照组(P <0.05),差异有统计学意义。结论:痛风康联合秋水仙碱治疗痛风性关节炎的临床疗效确切,中西医结合治疗能提高临床治疗的痊愈率,坚持长期服用痛风康能降低UA水平。
Objective:To observe the clinical effect of Tongfengkang(Wan)[痛风康(丸剂)]combined with colchicine in the treatment of gouty arthritis.Methods:According to the principle of random parallel control,80 patients were randomly divided into the treatment group and the control group.Both groups were given basic treatment of gouty arthritis,and colchicine was taken orally.The treatment group was supplemented with Tongfengkang(Wan)on the basis of the treatment in the control group.Erythrocyte deposition rate(ESR),hypersensitive C-reactive protein(CRP),blood uric acid(UA)and Visual analog scale(VAS)pain score were observed after 1 week of treatment.Colchicine was discontinued,and the treatment group continued to take Tongfengkang orally,and the blood uric acid changes were observed 1 month later.Results:After 1 week of treatment,the significant efficiency in the treatment group was 85.0%(34/40),which was higher than 72.5%(29/40)in the control group,but there was no significant difference between the two groups(P>0.05),the recovery rate in the treatment group was 60%(24/40),which was significantly higher than 37.5%(15/40)in the control group(P<0.05),the difference was statistically significant.After 1 week of treatment,VAS pain score,CRP and ESR levels in the treatment group were significantly lower than those in the control group(P<0.05).Serum uric acid in both groups were decreased to a certain extent,and there was no significant difference between the two groups after 1 week of treatment(P=0.40),but after 1 month of treatment,the serum uric acid level in the treatment group was significantly lower than that in the control group(P<0.05),and the difference was statistically significant.Conclusion:Tongfengkang combined with colchicine has a definite therapeutic effect on gouty arthritis.The combination of TCM and Western medicine can improve the recovery rate of clinical treatment,and the long-term administration of Tongfengkang can reduce the level of blood uric acid.
