摘要
目的探讨冷热圈套器内镜下黏膜切除术(cold snare endoscopic mucosal resection,CS-EMR)对直径6~10 mm结直肠息肉患者手术时间、完整切除率及并发症的影响。方法随机选取2021年1月—2022年12月苏州市相城人民医院收治的结直肠息肉患者300例为研究对象,以随机数表法将其分为A组和B组。A组150例患者给予行热圈套器内镜下黏膜切除术(endoscopic mucosal resection with thermal snare device,HS-EMR)治疗,B组150例患者给予CS-EMR术治疗。对比两组完全切除率、整块切除率、切除时间、切除息肉回收率与回盲部插管率、术后并发症发生率。结果两组切除病灶时间、息肉回收率和回盲部插管率比较,差异无统计学意义(P>0.05);两组整块、完全切除率和不同病理类型息肉完整切除率比较,差异无统计学意义(P>0.05);B组并发症发生率(1.33%)低于A组(6.00%),差异有统计学意义(χ^(2)=4.624,P<0.05)。结论将CS-EMR用于直径为6~10 mm结直肠息肉的治疗,整体效果与HS-EMR相当,但有着更低的出血、穿孔发生率,安全性更突出。
Objective To investigate the effect of cold snare endoscopic mucosal resection(CS-EMR)on the operation time,complete resection rate and complications in patients with colorectal polyps of 6-10 mm in diameter.Methods 300 patients with colorectal polyps admitted to Xiangcheng People's Hospital in Suzhou from January 2021 to December 2022 were randomly selected as the research subjects,and they were divided into group A and group B using a random number table method.150 patients in group A were treated with endoscopic mucosal resection with thermal snare device(HS-EMR),while 150 patients in group B were treated with CS-EMR.Compared the complete resection rate,whole block resection rate,resection time,polyp recovery rate,ileocecal intubation rate,and postoperative complication incidence between the two groups.Results There was no statistically significant difference in the resection time,polyp recovery rate,and ileocecal intubation rate between the two groups(P>0.05).There was no statistically significant difference between the two groups in terms of overall resection rate,complete resection rate,and complete resection rate of polyps of different pathological types(P>0.05).The incidence of complications in group B(1.33%)was lower than that in group A(6.00%),the difference was statistically significant(χ^(2)=4.624,P<0.05).Conclusion The use of CS-EMR for the treatment of colorectal polyps with a diameter of 6-10 mm has an overall effect comparable to that of HS-EMR,but with a lower incidence of bleeding and perforation,and a more prominent safety profile.
