摘要
目的探究缺血性卒中患者葡萄糖代谢和神经元自噬调亡水平与认知障碍的相关性。方法选择120例缺血性卒中患者,根据蒙特利尔认知能力评定量表(MoCA)评分将纳人研究的患者分为轻度认知障碍组(n=33)、重度认知障碍组(n=19)和认知无障碍组(n=68)。对比分析各组患者葡萄糖代谢和神经元自噬调亡水平,采用Pearson法分析葡萄糖代谢和神经元自噬调亡水平与认知障碍的相关性。结果重度认知障碍组患者的乳酸、血糖、L/P值、sFasL和sFas水平高于轻度认知障碍组和认知无障碍组(P<0.05);Bcl-2水平则低于轻度认知障碍组和认知无障碍组(P<0.05);而三组的丙酮酸水平和L/G值差异无统计学意义(P>0.05)。葡萄糖代谢与认知障碍呈负相关关系,与神经元自噬调亡水平呈正相关关系(P<0.05)。结论缺血性卒中患者血清中葡萄糖代谢和神经元自噬调亡水平与认知障碍均存在一定的相关性,可作为判断缺血性卒中患者认知障碍程度的指标。
Objective To explore the correlation between glucose metabolism,neuronal autophagy apoptosis and cognitive impairment in patients with ischemic stroke.Methods A total of 120 patients with ischemic stroke were selected and divided into mild cognitive impairment group(n=33),severe cognitive impairment group(n=19),and cognitive handicap group(n=68)according to the Montreal Cognitive Ability Rating Scale(MoCA).The levels of glucose metabolism and neuronal autophagy apoptosis in each group were compared and analyzed,and then Pearson method was used to analyze the correlation between glucosemetabolism and neuronal autophagy apoptosis and cognitive impairment.Results The lactic acid,blood glucose level,L/P value,sFasL and sFas in the severe cognitive impairment group were significantly higher than those in the mild cognitive impairment group and the cognitive barrier free group(P<0.05).The level of Bcl-2 was significantly lower than that of mild cognitive impairment group and cognitive barrier free group(P<0.05).There was no significant difference in pyruvate level and L/G value among the three groups(P>0.05).Glucose metabolism was negatively correlated with cognitive impairment,and positively correlated with neuron autophagy apoptosis level(P<0.05).Conclusion Serum glucose metabolism and neuronal autophagy apoptosis levels in patients with ischemic stroke are correlated with cognitive impairmentto someextent,whichcan be used as indicators to judge the degree of cognitive impairment in patients with ischemic stroke.
