摘要
甲状腺功能亢进症(简称甲亢),是甲状腺激素增多导致身体机能亢进的一类内分泌疾病,以心慌手抖、多食易饥、体质量减轻、眼球突出、烦躁易怒为主要临床表现。西医通常选用抗甲状腺药物、甲状腺切除术等方法治疗该病,虽有疗效,但存在需要长期服药、复发率高、手术创伤大、不良反应多等问题。中医将该病归为“瘿病”范畴。贾颖教授具有多年的临床、科研、教学经验,主要运用调和阴阳的方法治疗该病,临床效果显著。文章以阴阳为切入点,通过查阅文献并结合医案分析,浅析贾颖教授治疗“甲亢”过程中的点滴感悟。贾师认为,甲亢主要病机为阴虚阳亢,其中的关键原因为七情郁结,阴虚气弱是基本病变,阴损及阳是转折点。贾师在汲取前人经验及对赵尚华老先生所提出的治疗理念进行全面传承的基础上,对其进行中医辨证分型,分为肝郁痰结、阴虚阳亢、气阴两虚、阴损及阳,并把现代医疗知识融入阴阳之道,在辨证施治的过程中形成了自身的诊疗特色,实现了使患者机体阴阳稳态平衡的治疗目的。
Hyperthyroidism is a kind of endocrine disease caused by hyperfunction of the body due to increased thyroid hormone.The main clinical manifestations are palpitation and hand shaking,overeating and easy hunger,weight loss,eyeball protrusion,irritability and so on.In Western medicine,anti-thyroid medicines,thyroidectomy and other methods are used to treat the disease.Although those methods show curative effects,there are problems such as the need for long-term medication,high recurrence rate,large surgical trauma,and many adverse reactions.TCM classifies the disease into Ying(瘿)disease category.Professor JIA Ying has many years of clinical,scientific research and teaching experience.The professor mainly uses the method of harmonizing Yin(阴)and YangH(阳)to treat the disease,and has achieved remarkable clinical effects.In this article,taking Yin and Yang as the breakthrough point,the clinical experience of Professor JIA Ying in treating hyperthyroidism is briefly analyzed through consulting related literature and medical case analysis.Professor JIA believes that the main pathogenesis of hyperthyroidism is Yin deficiency and Yang hyperactivity,and that the key reason is emotional disorders.Professor JIA also believes that Yin deficiency and Qi weakness are the basic pathological changes,and that Yin damaging Yang is the turning point.Based on the previous experience and the comprehensive inheritance of the treatment concept proposed by Professor ZHAO Shanghua,Professor JIA divides the diease into the liver depression and phlegm accumulation type,the Yin deficiency and Yang hyperactivity type,the Qi and Yin both deficiency type,and Yin damaging Yang type.Professor JIA also integrates modern medical knowledge into the theory of Yin and Yang,forming her own diagnosis and treatment characteristics in the process of syndrome differentiation and treatment,and the therapeutic purpose of balancing Yin and Yang has obtained clinically.
