麝香通心滴丸改善冠脉微栓塞猪心功能及机制探讨 被引量:1

摘要 目的 分析麝香通心滴丸对冠脉微栓塞心肌的保护作用,并探讨其相关机制。方法 选用巴马小型猪18只,随机分为假手术对照组(SHAM组,n=6)、冠脉微栓塞模型组(CME组,n=6)、麝香通心滴丸干预组(SXTXDW组,n=6)。检测各组在不同时间,血清肌钙蛋白(TNI)、B型脑钠肽(BNP)、一氧化氮(NO)、一氧化氮及合成酶(NOS)、内皮素(ET-1)水平,心肌病理学,以及左心导管检查测定左心室收缩末压力(LVESP),左心室舒张末压力(LVEDP),左心室射血分数(LVEF)及心肌局部活动异常情况。结果 与SHAM组比较,CME组血清ET-1水平上升、NO、NOS水平下调(P<0.05),心肌组织可见明显微梗死灶、炎性细胞浸润;LVEDP明显升高,LVEF下降,前壁出现明显的局部活动减弱。与CME组比较,SXTXDW组血清ET-1表达水平下降、NO、NOS水平一定程度恢复(P<0.05),心肌组织梗死灶、炎性细胞浸润减轻,左心室功能改善(P<0.05)。结论 麝香通心滴丸可改善冠脉微栓塞心肌损伤,其机制可能与改善内皮功能、促进微血管修复相关。 Objective To analyze the protective effect of Shexiang Tongxin Dropping Pill(麝香通心滴丸)on coronary micro-embolization myocardium and explore its related mechanism.s Methods Eighteen Bama miniature pigs were randomly divided into the Sham-operated group(SHAM group,n=6),the coronary microembolism group(CME group,n=6)and the Shexiang Tongx-in Dropping Pill group(SXTXDW group,n=6).The serum levels of troponin(TNI),B-type brain natriuretic peptide(BNP),nitric oxide(NO),nitric oxide and synthase(NOS),endothelin(ET-1),myocardial pathology and left ventricular end systolic pressure(LVESP),left ventricular end diastolic pressure(LVEDP),left ventricular ejection fraction(LVEF)and local myocardi-um were measured by left heart catheterization.s Results Compared with those of the SHAM group,the serum level of ET-1 in the CME group increased,the levels of NO and NOS decreased(P<0.05),and the myocardial infarction and inflammatory cell infil-tration were obvious.LVEDP increased obviously,LVEF decreased and local activity of anterior wall decreased obviously.Com-pared with those of the CME group,the expression levels of serum ET-1 in the SXTXDW group decreased,the levels of NO and NOS recovered to some extent(P<0.05),myocardial infarction and inflammatory cell infiltration decreased and left ventricular function improved(P<0.05).n Conclusion Shexiang Tongxin Dropping Pill can improve myocardial injury caused by coronary mi-croembolization,and its mechanism might be related to improving endothelial function and promoting microvascular repair.
