摘要
目的:分析孕期血清游离雌三醇(uE3)、胎盘泌乳素(PL)及胰岛素样生长因子-1(IGF-1)对胎儿生长受限(FGR)的预测价值。方法:采用回顾性分析法,对2020年1月至2022年6月在某院系统产检,并住院分娩的孕妇共220例展开研究,其中正常妊娠组110例,FGR组110例。对两组孕妇的一般资料进行比较,并对两组孕妇不同时刻的血清uE3、PL、IGF-1水平进行比较。结果:与正常妊娠组相比,FGR组的年龄、孕前BMI、既往孕次、既往产次、既往剖宫产次、既往流产次无明显差异(P>0.05),FGR组的分娩孕周缩短,差异有统计学意义(P<0.05)。两组孕妇的血清uE3、PL、IGF-1水平均随孕周的增加而升高,且与正常妊娠组相比,FGR组孕妇孕20、24、28、32、36、37周的血清uE3、PL、IGF-1水平均降低,差异有统计学意义(P<0.05)。结论:孕中期、孕晚期的FGR孕妇血清uE3、PL、IGF-1水平均低于同妊娠阶段的正常妊娠妇女,对FGR具有一定的预测价值。
Objective:To analyze the predictive value of serum free estriol(uE3),placental prolactin(PL)and insulin-like growth factor-1(IGF-1)in fetal growth restriction(FGR)during pregnancy.Methods:A retrospective analysis was used to study 220 pregnant women who were systematically examined in Zhengzhou Maternal and Child Health Hospital from January 2020 to June 2022 and delivered in our hospital,including 110 normal pregnant women and 110 FGR women.The general data of the two groups of pregnant women were compared,and the serum uE3,PL,IGF-1 levels of the two groups of pregnant women at different times were compared.Results:Compared with the normal pregnancy group,there was no significant difference in age,pre pregnancy BMI,previous pregnancy times,previous delivery times,previous cesarean section times,and previous abortion times in the FGR group(P>0.05).The gestational weeks of delivery in the FGR group were shortened,and the difference was significant(P<0.05).The levels of serum uE3,PL and IGF-1 in the two groups increased with the increase of gestational weeks.Compared with the normal pregnancy group,the levels of serum uE3,PL and IGF-1 in FGR group decreased at 20 weeks,24 weeks,28 weeks,32 weeks,36 weeks and 37 weeks of pregnancy,with significance(P<0.05).Conclusion:The serum uE3,PL and IGF-1 levels of FGR pregnant women in the middle and late pregnancy are lower than those of normal pregnant women in the same pregnancy stage,which has certain predictive value for FGR.
