多体素质子磁共振波谱对伴抑郁情绪男性中重度OSAHS患者的诊断价值

摘要 目的探讨多体素质子磁共振波谱(1H-MRS)对伴抑郁情绪的阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的诊断价值。方法对20例有抑郁情绪的中重度OSAHS患者、20例无抑郁情绪的中重度OSAHS及性别、年龄相匹配的20例健康对照者的双侧额叶均行头颅多体素1H-MRS检查。分别记录额叶各代谢物比值,比较两两之间的差异,并观察是否有乳酸峰的出现。结果20例有抑郁情绪的中重度OSAHS患者额叶NAA/Cr、NAA/Cho较无抑郁情绪健康对照者降低。20例无抑郁情绪的中重度OSAHS患者额叶NAA/Cr、NAA/Cho(分别为较健康对照者降低,P<0.01。无抑郁情绪的中重度OSAHS患者Cho/Cr较健康对照者升高,P<0.05)。三组均未检测到Lac峰。结论多体素1H-MRS能敏感显示伴抑郁情绪OSAHS患者的脑代谢改变,为探讨其病理生理基础及针对性治疗提供客观的影像学依据。 Objective To investigate the clinical application value of multivoxel 1H magnetic resonance spectroscopy(^(1)H-MRS)in patients suffering from moderate to severe obstructive sleep apnea hypopnea syndrome(OSAHS)associated with depressive mood.Methods Twenty cases of moderate to severe OSAHS patients′nderwent depressive mood and 20 cases of moderate to severe OSAHS patients without depressive mood all were performed brain multivoxel ^(1)H-MRS examinations.20 cases of healthy who match with its gender and age also performed brain multivoxel ^(1)H-MRS examinations.The ratios of brain metabolites of frontal lobe was recorded and analyzed respectively.To observe whether the lactate(Lac)peak appeared or not.ResuIts In region of frontal lobe,the NAA/Cr,NAA/Cho of the 20 cases of moderate to severe OSAHS patients underwent depressive mood,was decreased compared with the healthy.The NAA/Cr,NAA/Cho of the 20 cases of moderate to severe OSAHS patients underwent depressive mood was decreased compared with 20 cases of moderate to severe OSAHS patients without depressive mood.The NAA/Cr,NAA/Cho of the 20 cases of moderate to severe OSAHS patients without depressive mood was decreased compared with the healthy,and the difierences were statistically significant,P<0.01.The Cho/Cr of the 20 cases of moderate to severe OSAHS patients without depressive mood was increased compared with the healthy,the difference was statistically significant,P<0.05.Lac peak was not detected in all.Conclusion Multivoxel 1H-MRS could demonstrate sensitively changes of brain metabolism in patients with obstructive sleep apnea hypopnea syndrome associated with depressive mood,and may provide objective imaging basis for exploring its pathophysiological basis and targeted treatment.
