跗骨窦切口与切开复位内固定术治疗SandersⅡ、Ⅲ型跟骨骨折的预后对比

摘要 目的:对比研究跗骨窦切口与切开复位内固定术治疗SandersⅡ、Ⅲ型跟骨骨折的临床效果及预后情况。方法:回顾性选取2020年1月到2022年7月南海区第五人民医院的62例SandersⅡ、Ⅲ型跟骨骨折患者的临床资料,根据手术方法的不同分为研究组与对照组,每组31例。研究组采用跗骨窦切口微创手术治疗,对照组采用切开复位内固定术治疗。比较两组手术时间、失血量、住院时间、伤口愈合时间及伤口并发症。于术后3个月进行随访,采用跟骨侧轴位X线片测量Bohler角、Gissane角,采用Maryland足部功能评分评估足部功能恢复情况。结果:与对照组相比,研究组的手术时间、住院时间、伤口愈合时间更短,失血量更少(P<0.05);对照组浅表感染、血肿等术后伤口并发症发生率25.81%(8/31)高于研究组3.23%(1/31)(P<0.05);末次随访时研究组Bohler角、Gissane角均大于对照组,Maryland评分高于对照组(均P<0.05)。结论:与切开复位内固定术相比,用跗骨窦切口微创手术治疗SandersⅡ、Ⅲ型跟骨骨折的效果较好,不仅可有效地改善患者的围手术期指标,还能降低其术后伤口并发症的发生率,促进其足踝功能的恢复。 Objective:To o compare the clinical effect and prognosis of SandersⅡandⅢcalcaneal fractures treated with tarsal sinus incision and open reduction and internal fixation.Methods:The clinical data of 62 patients with Sanders II and III calcaneal fractures from January 2020 to July 2022 in the Fifth People's Hospital of South China Sea District were retrospectively selected.62 patients were randomly divided into study group and control group according to different surgical methods,with 31 patients in each group.The study group was treated with minimally invasive surgery through the tarsal sinus incision,and the control group was treated with open reduction and internal fixation.The operation time,blood loss,hospital stay,wound healing time and wound complications were compared between the two groups.Follow up was carried out 3 months after the operation.Bohler angle and Gissane angle were measured by axial X-ray film of calcaneus side,and the recovery of foot function was evaluated by foot function score.Results:Compared with the control group,the operation time,hospital stay,wound healing time and blood loss in the study group were shorter(P<0.05);The incidence of postoperative wound complications such as superficial infection and hematoma in the control group 25.81%(8/31)was higher than that in the study group 3.23%(1/31)(P<0.05);At the last follow-up,Bohler angle and Gissane angle in the study group were larger than those in the control group,and Maryland score was higher than that in the control group(P<0.05).Conclusion:Compared with open reduction and internal fixation,minimally invasive surgery with tarsal sinus incision is more effective in the treatment of SandersⅡandⅢcalcaneal fractures.It can not only effectively improve the perioperative indicators of patients,but also reduce the incidence of wound complications after surgery,and promote the recovery of ankle function.
