摘要
目的观察经尺骨鹰嘴截骨入路联合远端双钢板固定治疗肱骨远端粉碎性骨折的临床效果。方法纳入2018年6月至2021年6月于本院行肱骨远端粉碎性骨折手术治疗的40例患者为研究对象,按照手术方式的不同将其分为对照组和观察组,每组20例。对照组行常规固定术,观察组行经尺骨鹰嘴截骨入路联合远端双钢板固定术。比较两组的治疗效果。结果观察组的手术时间、术后骨折愈合时间短于对照组,术中出血量少于对照组,切口长度小于对照组,差异具有统计学意义(P<0.05)。观察组的关节功能恢复总有效率高于对照组,差异具有统计学意义(P<0.05)。术后1年,两组的健康状况调查简表(SF-36)评分高于术前,且观察组高于对照组(P<0.05)。观察组的并发症总发生率显著低于对照组,差异具有统计学意义(P<0.05)。结论经尺骨鹰嘴截骨入路联合远端双钢板固定治疗肱骨远端粉碎性骨折的效果显著,可提升肘关节功能,加速术后康复进程,且安全性较高,值得临床推广并应用。
Objective To observe the clinical effect of ulna olecranon osteotomy approach combined with distal double plate fixation in the treatment of comminuted fracture of distal humerus.Methods A total of 40 patients who underwent surgical treatment of comminuted fracture of distal humerus in our hospital from June 2018 to June 2021 were included as the study objects and divided into control group and observation group according to different surgical methods,with 20 cases in each group.The control group underwent conventional fixation,and the observation group underwent ulna olecranon osteotomy approach combined with distal double plate fixation.The therapeutic effects of the two groups were compared.Results The operation time and postoperative fracture healing time in the observation group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,the incision length was smaller than that in the control group,and the differences were statistically significant(P<0.05).The total effective rate of joint function recovery in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).At 1 year after surgery,the MOS-Item Short Form Health Survey(SF-36)score in both groups was higher than that before surgery,and that in the observation group was higher than the control group(P<0.05).The total incidence of complications in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Ulna olecranon osteotomy approach combined with distal double plate fixation in the treatment of comminuted fracture of distal humerus has a significant effect,it can improve elbow joint function,and accelerate postoperative rehabilitation process,with high safety,which is worthy of clinical promotion and application.
