摘要
目的 探讨牙槽骨微穿孔术(micro-osteoperforations,MOPs)在成人拔牙正畸治疗中的临床应用效果。方法选取2021年1—12月于徐州市中心医院口腔科就诊的需拔除4颗第一前磨牙行正畸治疗的成人患者40例。患者排齐牙齿后,采用自身对照设计,将上颌左右侧随机分为研究组(行MOPs)和对照组(行传统正畸治疗)。分析比较2组拔牙间隙关闭时间、尖牙累计移动距离,并采用数字评分法(numerical rating scale,NRS)评估疼痛程度。结果 研究组拔牙间隙关闭时间为(3.42±0.19)个月,明显短于对照组[(6.78±0.40)个月],差异有统计学意义(t=-47.614,P <0.001)。关闭间隙治疗4、8、12周后,研究组的尖牙累计移动距离依次为(1.92±0.19)、(3.81±0.26)、(5.39±0.20)mm,对照组依次为(0.94±0.13)、(1.86±0.15)、(2.71±0.21)mm;相同治疗时间组间比较,差异均有统计学意义(均P <0.05)。2组关闭间隙治疗第1、7天的NRS评分较关闭间隙治疗前明显增高,差异均有统计学意义(均P <0.05);而关闭间隙治疗第14、28天的NRS评分与关闭间隙治疗前比较,差异均无统计学意义(均P> 0.05);且在关闭间隙治疗过程中,研究组与对照组NRS评分比较,差异均无统计学意义(均P> 0.05)。结论 与传统正畸治疗相比,MOPs可有效缩短成人拔牙正畸患者间隙关闭时间,加速尖牙移动,并无更明显的疼痛不适。
Objective To investigate the clinical application effects of micro-osteoperforations(MOPs)on adult orthodontic treatment with first premolars extraction. Methods Forty adult orthodontic patients with extraction of 4 first premolars were selected between January 2021 and December 2021 in the Department of Stomatology of Xuzhou Central Hospital. A self-control design was used. After aligning the teeth,the maxillary teeth were randomly divided into the research group(MOPs)and the control group(traditional orthodontic treatment). The duration of closing extraction space and distance of canine movement were compared,and the degree of pain and discomfort was assessed by the numerical rating scale(NRS). Results The time needed for closing extraction space in the research group was(3.42 ± 0.19)months,significantly shorter than that in the control group[(6.78 ± 0.40)months]. The difference was statistically significant(t =-47.614,P<0.001). At 4 weeks,8 weeks and 12 weeks after closing extraction space,the cumulative distances of the canine movement in the research group were(1.92 ± 0.19),(3.81 ± 0.26)and(5.39 ± 0.20)mm,while the distances in the control group were(0.94 ± 0.13),(1.86 ±0.15)and(2.71 ± 0.21)mm,respectively,and the differences between the two groups were statistically significant(P<0.05). At 1 day and 7 days after the beginning of closing extraction space,both groups reported higher levels of discomfort compared with the levels before closing extraction space,and the differences were statistically significant(P<0.05). At 14 days and 28 days,the patients had no obvious pain or discomfort(P>0.05). During the process of closing extraction space,the differences in NRS between the two groups were not statistically significant(P>0.05). Conclusion MOPs can effectively shorten the time of closing extraction space in adult orthodontic patients with tooth extraction and accelerate canine movement with no significant pain or discomfort compared with traditional orthodontic treatment.
