摘要
目的 构建COPD患者居家疾病管理风险清单,并验证风险清单的应用效果。方法 将58例住院COPD患者随机分为干预组和对照组各29例。对照组行常规护理与健康教育;干预组在常规护理基础上,制定基于情境识别的COPD患者居家疾病管理风险清单,用于患者管理,患者出院后3个月评价效果。结果 干预组健康知识得分显著高于对照组、再住院率显著低于对照组(均P<0.05);受访的8名医护人员对风险清单的作用予以肯定。结论 基于情境识别的COPD患者居家疾病管理风险清单的应用,有利于提高COPD患者健康知识水平、降低再住院率,提升疾病管理成效。
Objective To develop a risk management checklist for COPD patients in the home and to verify its application effect.Methods Fifty-eight hospitalized COPD patients were randomly assigned to an intervention group(n=29)and a control group(n=29).The control group received routine care and health education,while the intervention group,in addition to routine care,was provided with a context-awareness risk management checklist specifically tailored for COPD patients.The checklist was used for patient management,and its effect was evaluated three months after patient discharge.Results The intervention group had significantly higher disease-related knowledge and lower rate of readmission compared with the control group(both P<0.05).Eight healthcare professionals interviewed affirmed the role of the checklist.Conclusion The application of a context-awareness management risk checklist for COPD patients in home setting contributes to improving their disease-related knowledge,reducing readmission rate,and enhancing effect of disease management.
