摘要
目的探讨乙酰半胱氨酸联合红霉素治疗小儿支原体肺炎(MP)的疗效及对免疫功能的影响。方法选择2019年1月至2020年6月芜湖市第一人民医院收治的80例MP患儿为研究对象,采用随机数字表法分为观察组和对照组,各40例。对照组采用常规治疗+红霉素静脉注射,每次15 mg/kg,每日2次;观察组在对照组基础上加用乙酰半胱氨酸雾化吸入,3 ml乙酰半胱氨酸溶于5 ml 0.9%氯化钠溶液,两组均连续治疗7 d。比较两组临床相关指标、临床疗效,治疗前后炎症指标[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)]、免疫功能指标(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)),以及不良反应发生情况。结果观察组体温恢复时间、啰音消失时间、咳嗽持续时间、住院时间均短于对照组[(3.07±0.41)d比(3.48±0.79)d、(4.43±0.66)d比(5.09±0.80)d、(5.60±0.97)d比(6.51±1.18)d、(5.76±0.99)d比(6.86±1.04)d](均P<0.01)。观察组总有效率高于对照组[95.00%(38/40)比77.50%(31/40)](P<0.05)。治疗后,两组IL-6、TNF-α、CRP水平均低于治疗前,且观察组低于对照组(P<0.01);两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,且观察组高于对照组(P<0.01)。两组总不良反应发生率比较差异无统计学意义(P>0.05)。结论乙酰半胱氨酸联合红霉素治疗MP患儿的疗效显著,可有效调节炎症反应,改善免疫功能,促进患儿康复,且未增加不良反应发生率。
Objective To observe the effect and influence on immunity of acetylcysteine combined with erythromycin in treating children with mycoplasma pneumonia(MP).Methods A total of 80 children with MP treated in Wuhu No.1 People′s Hospital from Jan.2019 to Jun.2020 were included and divided into an observation group and a control group according to the random number table method,40 cases in each group.The control group received routine treatment+erythromycin(intravenous injection,15 mg/kg each time,twice a day),and the observation group was added acetylcysteine(atomized inhalation,3 ml acetylcysteine dissolved in 5 ml 0.9%sodium chloride solution)based on the control group′s regimen,both groups were treated continuously for 7 days.The therapeutic effect and the clinical indexes of the two groups were compared,the inflammatory indexes[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)],immune function indexes(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))before and 7 days after treatment,as well as the occurrence of adverse reactions of the two groups were compared.Results The temperature recovery time,rale disappearance time,cough duration and hospital stay in the observation group were shorter than those in the control group[(3.07±0.41)d vs(3.48±0.79)d,(4.43±0.66)d vs(5.09±0.80)d,(5.60±0.97)d vs(6.51±1.18)d,(5.76±0.99)d vs(6.86±1.04)d](all P<0.01).The total effective rate of the observation group was higher than that of the control group[95.00%(38/40)vs 77.50%(31/40)](P<0.05).After treatment,the levels of IL-6,TNF-αand CRP in the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.01).The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the two groups were higher than those before treatment,and the observation group was higher than the control group(P<0.01).There was no significant difference in the incidence of total adverse reactions between the two groups(P>0.05).Conclusion Acetylcysteine combined with erythromycin is effective in
