摘要
癌因性疲乏(CRF)是最困扰癌症患者的症状之一,其机制尚不明确,且以中枢性兴奋类药物和皮质醇等药物为主的西药治疗亦存在诸多局限。中药具有多途径、多靶点的优势,可通过抗炎、调节代谢水平、抗氧化、调节下丘脑-垂体-肾上腺轴和5-羟色胺水平等途径改善CRF。当前中药改善CRF的机制研究以动物体力改变为主要观察指标,重视外周血液生化指标的改变,但仍存在中药有效成分不明确、实验动物模型单一等问题。未来的研究可围绕中药改善CRF躯体、认知、情感等多维度开展,借助网络药理学和“病证结合”的动物模型,对中药改善CRF的机制进行深入至分子细胞学水平的研究。
Cancer-related fatigue(CRF)is one of the most troubling symptoms of cancer patients,the mechanism of which is still unclear,and there are also many limitations in western medicine treatment based on central stimulant drugs and cortisol.Traditional Chinese medicine(TCM)has the advantages of multiple pathways and multiple targets,which can improve CRF by anti-inflammation,regulating metabolic level,anti-oxidation,regulating hypothalamic-pituitary-adrenal axis and 5-hydroxytryptamine level.At present,the mechanism of TCM in improving CRF is based on animal physical strength as the main outcome measures,and the changes in peripheral blood biochemical indicators are emphasized.However,there are still some problems such as unclear effective TCM ingredients and single experimental animal model.Future research can focus on the TCM improvement of multidimensional aspects such as CRF soma,cognition and emotion,and in-depth research on the mechanism of TCM improving CRF at the molecular cytology levels should be conducted with the help of network pharmacology and animal model combining disease and syndrome.
