摘要
目的探讨失效模式与效应分析(failure mode and effect analysis,FMEA)在妇科日间手术护理安全管理中的应用效果。方法选取温州医科大学附属第一医院2022年1月至6月实施FMEA前收治的200例患者为对照组,2022年7月至12月实施FMEA后收治的200例患者为观察组。对照组采用传统模式对日间手术患者进行安全管理,观察组应用FMEA方法对围手术期流程中存在的缺陷事件进行系统分析,计算风险优先指数(risk priority number,RPN),查找高危环节,制定并实施改进措施。结果实施FMEA后,观察组患者术前准备不完善、手术风险评估不足、身份查对错误、计划外中止手术、并发症防治不当的RPN值均显著低于对照组(P<0.05)。观察组患者的术前准备完善率、满意度均显著高于对照组(P<0.05),术后并发症发生率及非计划再入院率均显著低于对照组(P<0.05)。结论FMEA在妇科日间手术患者中的应用,有助于规范围手术期工作流程,保证日间手术护理质量和安全。
Objective To explore the application effect of failure mode and effect analysis(FMEA)in nursing safety management of gynecological day surgery.Methods A total of 200 patients admitted to the First Affiliated Hospital of Wenzhou Medical University from January to June 2022 before the implementation of FMEA were selected as control group,and 200 patients admitted after the implementation of FMEA from July to December 2022 were selected as observation group.The control group adopted the traditional mode of safety management for day surgery patients.The observation group applied FMEA method to systematically analyze the defect events existing in perioperative process,calculate the risk priority number(RPN),identified high-risk links,and developed and implemented improvement measures.Results After the implementation of FMEA,the RPN values of patients in observation group were significantly lower than those in control group,such as incomplete preoperative preparation,inadequate surgical risk assessment,wrong identity check,unplanned termination of surgery,and improper prevention and treatment of complications(P<0.05).The perfect rate of preoperative preparation and satisfaction in observation group were higher than those in control group(P<0.05),incidence of postoperative complications and unplanned readmission rate in observation group were lower than those in control group(P<0.05).Conclusion The application of FMEA in gynecological day surgery patients is helpful to standardize the perioperative work process and ensure the quality and safety of day surgery nursing.
