培元固神针法联合目标导向功能训练对老年中风恢复期肢体功能障碍患者肢体功能及日常生活能力的影响

摘要目的:分析培元固神针法联合目标导向功能训练对老年中风恢复期肢体功能障碍患者肢体功能及日常生活能力的影响。方法:选取2022年1~12月我院收治的老年中风恢复期肢体功能障碍患者72例为研究对象,通过随机数字表法分为对照组和研究组各36例。对照组采用基础康复护理及目标导向功能训练,研究组在对照组基础上采用培元固神针法干预。对比两组患者干预后肌力分级优良率、Brunnstrom六期评估法(BRS)分级优良率和干预后不良事件发生情况,评估患者日常生活能力和干预后的安全性,比较两组患者干预前后Barthel指数量表、中风患者神经功能缺损标准(CSS)、斯堪地那维亚卒中量表(SSS)和蒙特利尔认知评估量表(MoCA)评分以及血清指标水平,分别评估患者神经功能恢复、疾病导致症状的轻重程度、认知功能以及临床疗效。结果:干预后,研究组肌力分级和BRS分级优良率分别为77.78%和83.33%,均显著高于对照组的55.56%和58.33%(P<0.05);干预后,研究组的Bartuel评分、SSS评分和MoCA评分均显著高于对照组,研究组的CSS评分,NGF、NT-3和BDNF水平均显著低于对照组(P<0.05);两组患者干预后安全性对比无统计学意义(P>0.05)。结论:对于老年中风恢复期肢体功能障碍患者而言,采用培元固神针法联合目标导向功能训练可有效促进患者肌肉运动功能、肢体运动能力、神经功能及认知功能恢复,改善日常生活能力,提高临床治疗效果,值得临床推广。 Objective:To analyze the effect of Peiyuan Gushen Acupuncture Therapy combined with goal-oriented functional training on limb function and activities of daily living of elderly stroke patients with limb dysfunction during recovery period.Methods:A total of 72 elderly stroke patients who had limb dysfunction during recovery period and were admitted to the hospital from January to December 2022 were selected as the research subjects.They were divided into the control group and the study group by random number table method,with 36 cases in each group.The control group received basic rehabilitation nursing and goal-oriented functional training,while the study group received Peiyuan Gushen Acupuncture Therapy on this basis.The excellent and good rates of muscle strength and Brunnstrom recovery stages(BRS),the incidence of adverse events after intervention were compared between the two groups.The patients’activities of daily living and safety,Barthel index score,Chinese Stroke Scale(CSS)score,Scandinavian Stroke Scale(SSS)score,Montreal Cognitive Assessment Scale(MoCA)score,the levels of serum indicators,and clinical efficacy were compared between the two groups before and after the intervention.Results:After intervention,the excellent and good rates of muscle strength grading and BRS grading in the study group(77.78%and 83.33%)were significantly higher than those in the control group(55.56%and 58.33%)(P<0.05).After intervention,Barthel index scores,SSS scores and MoCA scores of the study group were significantly higher than those of the control group.The CSS scores and the levels of NGF,NT-3 and BDNF were significantly lower than those of the control group(P<0.05).There was no statistically significant difference in safety between the two groups after intervention(P>0.05).Conclusion:For elderly stroke patients with limb dysfunction during the recovery period,Peiyuan Gushen Acupuncture Therapy combined with goal-oriented functional training can effectively promote the recovery of muscle motor function,limb motor abil
