摘要
目的:检索、评价和总结肥胖病人外周静脉置管的最佳证据。方法:系统检索UpToDate、Best Practice(BMJ)、美国指南网、加拿大安大略注册护士协会网、苏格兰学院际指南网、医脉通指南网、the Cochrane Library、PubMed、中国生物医学文献数据库、万方数据库、中国知网、美国输液护士协会中关于肥胖病人外周静脉置管的最佳证据。包括指南、临床决策、专家共识、证据总结、系统评价及原始研究。检索时限为建库至2022年6月30日,由2名研究者独自完成纳入文献的质量评价后,并结合专业人士的判断,进行证据提取及整合。结果:共纳入8篇文献,其中1篇指南、2篇临床决策、2篇证据总结、1篇系统评价和2篇原始研究。总结了困难因素、置管前评估、设备选择、静脉选择、导管选择、人员资质、置管角度、干预策略、穿刺后评估及质量控制共10个方面21条证据。结论:总结的肥胖病人外周静脉置管最佳证据,可为护士实施肥胖病人外周静脉血管穿刺提供循证依据,以提升肥胖病人外周静脉置管成功率,提升病人舒适度、满意度及护理质量。
Objective:To retrieve,evaluate,and summarize the best evidence on peripheral intravenous catheterization in obese patients.Methods:Guidelines,clinical decision-making,expert consensus,evidence summaries,systematic reviews,and original research on the best evidence of peripheral intravenous catheterization in obese patients were retrieved from UpToDate,Best Practice(BMJ),American Guidelines Network,Canadian Ontario Registered Nurses Association Network,Scottish Intercollegiate Guidelines Network,Yimaitong Guidelines Network,the Cochrane Library,PubMed,CBM,WanFang Database,China National Knowledge Infrastructure(CNKI),and the American Infusion Nurses Association.The search time limit was from establishing the database to June 30,2022.Two researchers independently completed the quality evaluation,extracting and integrating evidence based on professionals′judgment.Results:A total of 8 articles were included,including 1 guideline,2 clinical decision-making articles,2 evidence summaries,1 systematic review,and 2 original studies.A total of 21 pieces of evidence were summarized in 10 aspects,including difficult factors,pre-catheterization assessment,equipment selection,vein selection,catheter selection,personnel qualifications,cannulation angle,intervention strategy,post-puncture assessment,and quality control.Conclusion:The summarized best evidence for peripheral intravenous catheterization in obese patients could provide the evidence-based basis for nurses to perform peripheral intravenous catheterization in obese patients,improve the success rate of peripheral intravenous catheterization in obese patients,and improve patients′comfort,satisfaction,and quality of nursing.
