摘要
目的 口腔黏膜下纤维性变(OSF)相关口腔鳞状细胞癌(OSCC)患者唾液菌群、细胞角蛋白19片段(Cyfra21-1)及过氧化氢酶(CAT)含量变化及与其临床病理特点的关系。方法 选择OSF相关OSCC患者96例,同时选择同期OSF无相关OSCC患者96例。收集患者资料,包括患者肿瘤部位、肿瘤直径、分化程度、淋巴结转移、临床分期。采集患者清晨空腹状态下无刺激性混合唾液4 mL,检测唾液菌群分布以及Cyfra21-1、CAT含量变化,比较不同病理特点OSCC患者唾液中Cyfra21-1、CAT含量。结果 OSF相关OSCC患者唾液中CYFRA21-1、CAT含量显著高于OSF无相关OSCC患者(P<0.05)。96例OSF相关OSCC患者唾液中共分离出104株菌株,其中奈瑟氏菌群、链球菌属占比较高,分别为14.42%、13.46%。OSF相关OSCC患者唾液中CYFRA21-1含量与患者性别、年龄、BMI、肿瘤部位、分化程度无关(P>0.05),与临床分期、淋巴结转移有关(P<0.05);CAT含量与患者性别、年龄、BMI、肿瘤部位、分化程度、临床分期、淋巴结转移均无关(P>0.05)。ROC曲线分析结果显示,唾液中CYFRA21-1、CAT含量诊断OSF相关OSCC的AUC分别为0.795、0.733,均有较好的诊断价值(P<0.05)。结论 OSF相关OSCC患者唾液菌群主要为奈瑟氏菌群、链球菌属,其唾液中CYFRA21-1、CAT含量较高,且与患者病理特点密切相关。
Objective To explore the changes of salivary floras,cytokeratin 19 fragment(Cyfra21-1)and catalase(CAT)in patients with oral submucosal fibrosis(OSF)related oral squamous cell carcinoma(OSCC)and their relationship with pathological characteristics.Methods A total of 96 patients with OSF-related OSCC and 96 OSF patients without OSCC were enrolled.The case data of patients were collected,including tumor sites,tumor diameter,differentiation degree,lymph node metastasis and clinical staging.The 4 mL of non-irritating mixed saliva was collected in the morning under fasting state to detect the changes in distribution of salivary floras,Cyfra21-1 and CAT.The levels of Cyfra21-1 and CAT in saliva of OSCC patients with different pathological characteristics were compared.Results The levels of CYFRA21-1 and CAT in saliva in patients with OSF-related OSCC were significantly higher than those in OSF patients without OSCC(P<0.05).In the 96 patients with OSF-related OSCC,there were 104 strains of pathogens,mainly including Neisseria and Streptococcus(14.42%,13.46%).CYFRA21-1 level in saliva was not correlated with gender,age,BMI,tumor sites or differentiation degree(P>0.05),but correlated with clinical staging and lymph node metastasis(P<0.05).CAT level was not correlated with gender,age,BMI,tumor sites,differentiation degree,clinical staging or lymph node metastasis(P>0.05).The results of ROC curve analysis showed that AUC values of CYFRA21-1 and CAT in the diagnosis of OSF-related OSCC were 0.795 and 0.733,which were of good diagnostic value(P<0.05).Conclusion The main salivary pathogens are Neisseria and Streptococcus in patients with OSF-related OSCC.The levels of CYFRA21-1 and CAT in saliva are high,which are closely related to pathological characteristics of patients.
