摘要
探讨奈维在预防食管癌切除术后吻合口瘘中的作用。回顾性分析235例接受“微创食管癌切除术(胃代食管、左颈吻合)”治疗的食管癌患者的资料。其中117例患者采用单纯器械吻合,118例患者在器械吻合后采用奈维包埋吻合口,对比两组患者术后吻合口瘘、吻合口狭窄、颈部感染的发生率和术后住院时间、住院总费用。结果显示,奈维包埋吻合口组患者较单纯器械吻合组患者术后吻合口瘘发生率降低8.6%、平均术后住院时间缩短近2 d、平均住院总费用减少3984元,差异有统计学意义(P值均<0.05);两组患者术后颈部感染及吻合口狭窄的发生率差异无统计学意义(P>0.05)。结果表明,奈维包埋食管-胃吻合口区域可预防吻合口瘘的发生,具有临床意义。
To investigate the role of Neovel in the prevention of anastomotic leakage after esophageal cancer surgery,The clinical data of 235 patients with esophageal cancer who underwent"minimally invasive esophagectomy(gastric replacement of esophagus and left neck anastomosis)"were retrospectively analyzed.Among them,117 patients were treated with simple instrument anastomosis,and 118 patients were treated with Neovellle embedding anastomosis after simple instrument anastomosis.The incidence of postoperative anastomotic leakage,anastomotic stenosis,neck infection,postoperative hospital stay and total hospitalization expenses were compared between the two groups.The results showed that the incidence of postoperative anastomotic leakage in the Neovellle embedding anastomosis group was 8.6%lower than that in the simple instrument anastomosis group,the average postoperative hospital stay was shortened by nearly 2 days,and the average total cost of hospitalization was reduced by 3984 yuan,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative neck infection and anastomotic stenosis between the two groups(P>0.05).The results showed that Neovel wrapping the esophagogastric anastomosis area could prevent the occurrence of anastomotic leakage and had clinical significance.
