摘要
目的 探究3D打印截骨导板在膝关节周围恶性骨肿瘤中的应用效果。方法 前瞻性选取2020年1月至2022年12月河南省洛阳正骨医院收治的60例膝关节周围恶性骨肿瘤患者为研究对象,按随机数表法分为研究组(30例)与对照组(30例),对照组接受常规截骨肿瘤膝关节假体节置换术治疗,研究组接受3D打印截骨导板辅助下肿瘤膝关节假体置换术治疗,比较两组手术相关指标、手术精度、疗效、膝关节功能(KSS评分)、关节活动度(ROM)与术后并发症情况。结果 研究组术中失血量、术后引流量少于对照组,手术时长短于对照组,FFC角差值、HKA角差值大于对照组(P<0.05)。研究组治疗总有效率高于对照组(P<0.05)。两组出院时、术后3个月KSS评分与术前相比均升高,且术后3个月高于出院时(P<0.05),两组出院时膝关节ROM低于术前,术后3个月高于术前及出院时(P<0.05)。两组术后并发症发生比较差异无统计学意义(P>0.05)。结论 3D打印截骨导板辅助肿瘤膝关节假体置换术治疗膝关节周围恶性骨肿瘤效果较好,可有效优化手术相关指标,缩短手术时间,减少出血量,且安全性好。
Objective To explore the application effect of 3D printed osteotomy guide plate in malignant bone tumors around the knee joint.Methods From January 2020 to December 2022,60 patients with malignant bone tumors around the knee joint were prospectively selected and divided into study group(30 cases)and control group(30 cases)according to the random number ranking method.The control group was treated with conventional osteotomy and tumor prosthesis replacement,while the study group was treated with 3D printed osteotomy guide assisted by tumor knee prosthesis replacement.Surgical related indicators,surgical accuracy,efficacy,knee society score(KSS score),joint range of motion(ROM)and postoperative complications were compared.Results The intraoperative blood loss and postoperative drainage volume in study group were less than those in control group,and the duration of surgery was longer than that in control group,the FFC angle difference and HKA angle difference were larger than those in control group(P<0.05).The total effective rate of treatment in study group was higher than that in control group(P<0.05).The KSS score at discharge and 3 months after surgery in both groups increased compared to before surgery,and it was higher at 3 months after surgery than at discharge(P<0.05).The knee joint ROM at discharge in both groups was lower than before surgery,and it was higher at 3 months after surgery than before surgery and after discharge(P<0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion The 3D printed osteotomy guide plate assisted knee joint prosthesis replacement surgery for malignant bone tumors around the knee joint has a good effect,which can effectively optimize surgical related indicators,reduce surgical time and bleeding,and have good safety.
