摘要
目的:观察通脉舒筋针法联合痛消合剂外敷治疗腰椎术后残余腰背痛的临床疗效。方法:选取96例行腰椎间融合术治疗且术后3个月仍有腰背痛的患者,按照随机数字表法分为对照组和观察组各48例。对照组给予塞来昔布胶囊治疗,观察组给予通脉舒筋针法联合痛消合剂外敷治疗。2组均治疗2周,随访3个月。比较2组临床疗效,Oswestry功能障碍指数(ODI)、视觉模拟评分法(VAS)评分,疼痛因子[神经肽Y (NPY)、P物质(SP)、前列腺素E2 (PGE2)]、氧化应激状态指标[超氧化物歧化酶(SOD)、总抗氧化能力(T-AOC)、丙二醛(MDA)]水平,以及复发率,记录2组不良反应。结果:治疗后,观察组临床疗效总有效率91.11%,高于对照组75.00%(P<0.05)。2组ODI、VAS评分均较治疗前降低(P<0.05),观察组ODI、VAS评分均低于对照组(P<0.05)。2组血清NPY、SP、PGE2、MDA水平均较治疗前降低(P<0.05),观察组血清NPY、SP、PGE2、MDA水平均低于对照组(P<0.05)。2组血清SOD、T-AOC水平均较治疗前升高(P<0.05),观察组血清SOD、T-AOC水平均高于对照组(P<0.05)。治疗过程中,对照组出现1例恶心,观察组出现1例局部酸痛,不良反应均轻微。随访3个月,观察组未见复发,对照组复发率25.00%,2组比较,差异无统计学意义(P>0.05)。结论:通脉舒筋针法联合痛消合剂外敷有利于减轻腰椎术后残余腰背痛,改善腰椎功能,安全性好,复发率低。
Objective:To observe the clinical effect of Tongmai Shujin acupuncture method combined with external application with Tongxiao Mixture for residual low back pain after lumbar interbody fusion.Methods:A total of 96 cases of patients undergoing lumbar interbody fusion and still suffered from low back pain in 3 months after surgery were divided into the control group and the observation group according to the random number table method,with 48 cases in each group.The control group was treated with Celecoxib Capsules,and the observation group was treated with Tongmai Shujin acupuncture method combined with external application with Tongxiao Mixture.Both groups were treated for 2 weeks and followed up for 3 months.The clinical effects,the scores of Oswestry Disability Index(ODI) and Visual Analogue Scale(VAS),the levels of pain factors [neuropeptide Y(NPY),substance P(SP),prostaglandin E2(PGE2)] and oxidative stress indexes [superoxide dismutase(SOD),total antioxidant capacity(T-AOC),malondialdehyde(MDA)],and the recurrence rates were compared between the two groups.The adverse reactions in the two groups were recorded.Results:After treatment,the total clinical effective rate was91.11% in the observation group,higher than that of 75.00% in the control group(P<0.05).The scores of ODI and VAS in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of ODI and VAS in the observation group were lower than those in the control group(P<0.05).The levels of NPY,SP,PGE2,and MDA in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of NPY,SP,PGE2,and MDA in serum in the observation group were lower than those in the control group(P<0.05).The levels of SOD and TAOC in serum in the two groups were increased when compared with those before treatment(P<0.05),and the levels of SOD and T-AOC in serum in the observation group were higher than those in the control group(P<0.05).During treatment,there was 1 case of nausea in the control group a
