摘要
目的探讨甲状腺乳头状癌(papillary thyroid carcinoma,PTC)患者程序性细胞死亡受体-1(PD-1)/程序性细胞死亡配体-1(PD-L1)的表达情况,分析其与患者临床病理特征及预后的关系。方法收集191例接受手术切除治疗的PTC患者,术中取癌组织及癌旁组织,采用免疫组化法检测组织中PD-1、PD-L1水平,分析不同临床病理特征患者间的PD-1、PD-L1表达情况。随访2年,采用单因素及多因素Logistic回归,分析PD-1、PD-L1阳性表达与PTC患者预后的关系。结果免疫组化结果显示,PTC患者癌组织中PD-1、PD-L1阳性表达率分别为39.27%、53.40%,高于癌旁组织(15.18%、22.51%),差异有统计学意义(χ^(2)=27.958、38.695,均P<0.001)。单因素分析结果显示:PD-1、PD-L1阳性表达与TNM分期、淋巴结转移有关(P<0.05),与性别、年龄、肿瘤大小无关(P>0.05)。随访2年期间,31例患者复发;Logistic回归分析结果显示,TNM分期高、淋巴结转移、PD-1及PD-L1阳性表达均为影响PTC患者预后的独立危险因素(P<0.05)。结论PTC患者癌组织中PD-1、PD-L1呈高表达,不同临床病理特征患者的PD-1、PD-L1阳性表达存在差异,两者可作为评估PTC预后的参考指标。
Objective Toexplore the expressions of programmed cell death receptor-1(PD-1)/programmed cell death ligand-1(PD-L1)in patients with papillary thyroid carcinoma(PTC),and to analyze their relationship with clinicopathological characteristics and prognosis of patients with PTC.Methods A total of 191 patients diagnosed with papillary thyroid carcinoma(PTC)and undergoing surgical resection were included in this study.Both carcinoma tissues and adjacent para-carcinoma tissues were collected intraoperatively.Immunohistochemistry was performed to assess of PD-1 and PD-L1,and the expressions of PD-1 and PD-L1 analyzed in relation to various clinicopathological characteristics of the patients.A two-year follow-up period was conducted to investigate the association between PD-1,PD-L1 expression,and the prognosis of PTC patients using univariate and multivariate logistic regression analysis.Results The results of immunohistochemistry showed that positive expression rates of PD-1 and PD-L1 in carcinoma tissues were 39.27%and 53.40%,higher than those in para-carcinoma tissues(15.18%,22.51%),the difference was statistically significant(χ^(2)=27.958,38.695,P<0.001).The results of univariate analysis showed that positive expressions of PD-1 and PD-L1 were related to TNM staging and lymph node metastasis(P<0.05),but not related to gender,age or tumor size(P>0.05).During 2 years of follow-up,there were 31 recurrence cases.The results of Logistic regression analysis showed that high TNM staging,lymph node metastasis,positive expressions of PD-1 and PD-L1 were independent risk factors of prognosis in PTC patients(P<0.05).Conclusions The expression levels of PD-1 and PD-L1 in carcinoma tissue are increased in PTC patients.Variations in PD-1 and PD-L1 exist amongst patients with diverse clinicopathological attributes.These parameters can serve as reliable reference indicators to assess the prognosis of PTC.
