摘要
目的分析放大内镜联合蓝激光成像技术(ME-BLI)诊断幽门螺杆菌根除后胃早癌的准确性,并评价其临床应用价值。方法以2019~2022年武汉市第一医院收治的90例经幽门螺杆菌根除治疗后1~2年,幽门螺杆菌检测为阴性,普通白光内镜检查疑似胃黏膜病变的患者为研究对象。所有患者行ME-BLI检查,并以病理学检查结果作为判断金标准,对比普通白光内镜和ME-BLI对胃早癌的诊断效率。结果62例患者经病理学检测确诊为胃早癌。ME-BLI对幽门螺杆菌根除后胃早癌的诊断敏感度、特异度、阳性预测值、阴性预测值和总体准确率均明显高于普通白光内镜,差异均有统计学意义(均P<0.05)。与普通白光内镜相比,ME-BLI对于胃黏膜病变的边界线(DL)、不规则黏膜微血管(MV)、不规则的表面微结构(MS)有更好的显示,差异均有统计学意义(均P<0.01)。结论ME-BLI对幽门螺杆菌根除后胃早癌具有良好的诊断价值,有助于提高胃早癌的临床诊断效率。
Objective To analyze the accuracy of magnifying endoscopy combined with blue laser imaging(ME-BLI)in the diagnosis of early gastric cancer after Helicobacter pylori eradication,and to evaluate its clinical application value.Methods Ninety patients admitted to Wuhan No.1hospital from 2019to 2022were included.Helicobacter pylori eradication were conducted in all patients.Suspected gastric mucosal lesions were examined by general white-light endoscopy and ME-BLI was performed.Pathological examination was the gold standard.The diagnostic efficiencies of conventional white light endoscopy and ME-BLI were compared.Results Eventually 62patients were diagnosed with early gastric cancer.The diagnostic sensitivity,specificity,positive predictive value,negative predictive value and overall accuracy of ME-BLI for early gastric cancer after Helicobacter pylori eradication were significantly higher than those of ordinary white light endoscopy,the difference was statistically significant(P<0.05).Compared with common white light endoscopy,ME-BLI showed better borderline(DL),irregular microvascular pattern(MV)and irregular microsurface pattern(MS)of gastric mucosal lesions(P<0.01).Conclusion ME-BLI has satisfying diagnostic value in the diagnosis of early gastric cancer after eradication of Helicobacter pylori,which is helpful in improving the clinical diagnosis efficiency of early gastric cancer.
