摘要
目的分析阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者不同睡眠体位及翻身频率特点,进一步探讨自动正压通气(automatic positive airway pressure,APAP)对OSAHS患者睡眠体位及翻身频率影响。方法选取在我院行多导睡眠监测(polysomnography,PSG)且符合入选标准的OSAHS患者及正常人群共149例,分析各组不同睡眠体位占总睡眠时间比例及翻身频率特点,OSAHS患者中同期行APAP治疗共58例,统计学分析治疗前、后睡眠体位及翻身频率变化。结果OSAHS患者翻身频率较正常组均明显提高;仰卧位时间占比中,重度OSAHS患者高于轻、中度患者,左、右卧位时间占比中,各组间无明显差异。正常组内仰卧位时间占比高于左、右卧位,轻度组内仰卧位时间占比高于左卧位,中度组内仰卧位及右侧卧位时间占比高于左侧卧位,重度组内不同睡眠体位时间占比均存在差异。经APAP治疗后,OSAHS患者翻身频率均明显降低,轻、中度OSAHS患者左侧卧位时间占比升高,右侧卧位时间占比下降。结论重度OSAHS患者仰卧位时间占比较高,随着OSAHS病情加重,患者右侧卧位时间占比逐渐增加,为减轻睡眠阻塞程度,OSAHS患者睡眠翻身频率会明显提高。APAP治疗不仅降低OSAHS患者翻身频率,还能够调整轻、中度患者侧卧位睡姿更加均衡。
Objective To analyze the characteristics of different sleep positions and turnover frequency in patients with obstructive sleep apnea hypopnea syndrome(OSAHS)and furtherly explore the effects of automatic positive airway pressure(APAP)on sleep position and turning frequency of OSAHS patients.Methods A total of 149 OSAHS patients and normal people who underwent polysomnography(PSG)monitoring in our hospital and met the requirements were selected,and the proportion of different sleep positions in the total sleep time and the characteristics of turning over frequency in each group were analyzed.A total of 58 OSAHS patients underwent APAP treatment at the same time.The changes of sleep position and turn over frequency before and after treatment were statistically analyzed.Results The turnover frequency of patients with OSAHS was significantly higher than that of normal group.The proportion of time in supine position was moderate,while the proportion of time in left and right decubitus position was moderate,and there was no significant difference among all groups.The proportion of time in supine position in normal group was higher than that in left and right decubitus,the proportion of time in supine position in mild group was higher than that in left decubitus,the proportion of time in supine position and right decubitus in moderate group was higher than that in left decubitus,the proportion of time in different sleep positions in severe group was different.After APAP treatment,the turnover frequency of patients with OSAHS was significantly reduced,and the proportion of time in left decubial position was increased and the proportion of time in right decubial position was decreased in patients with mild and moderate OSAHS.Conclusions Patients with severe OSAHS spend more time in supine position.With the aggravation of OSAHS,the proportion of time in right decubial position gradually increases.In order to relieve the degree of sleep obstruction,the sleep turning frequency of OSAHS patients will be significantly acceler
