摘要
目的:研究超声引导下微波射频消融治疗甲状腺结节的临床疗效及安全性.方法:选择2021年6月至2022年7月本院收治的68例甲状腺结节患者为研究对象,根据手术方式分为观察组(n=35,超声引导下微波射频消融术)和对照组(n=33,外科常规手术),对比两组治疗效果.结果:治疗3个月后两组游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)指标均下降,促甲状腺激素(TSH)指标上升;且观察组的FT3、FT4指标下降程度小于对照组,TSH升高程度小于对照组,差异有统计学意义(P<0.05).治疗后两组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平均降低,且观察组TNF-α、IL-6、CRP水平降低幅度大于对照组,差异有统计学意义(P<0.05).观察组并发症发生率(8.57%)低于对照组(30.30%),差异有统计学意义(P<0.05).结论:在甲状腺结节的治疗中使用超声引导下微波射频消融术,能有效避免甲状腺功能减退,降低炎性因子水平,同时保证了安全性,值得临床推广使用.
Objective:To investigate the clinical efficacy and safety of ultrasound-guided microwave radiofrequency ablation in the treatment of thyroid nodules.Methods:A total of 68 patients with thyroid nodule admitted to our hospital from June 2021 to July 2022 were selected.According to different surgical methods,they were divided into the observation group(n=35,ultrasound-guided microwave radiofrequency ablation)and the control group(n=33,routine surgical operation).Compare the treatment effects between the two groups.Results:After 3 months of treatment,free triodothyronine(FT3)and free tetraiodothyronine(FT4)indexes of the two groups decreased,while thyroid-stimulating hormone(TSH)indexes increased.Moreover,the decrease degree of FT3 and FT4 indexes of the observation group was less than that of the control group,and the increase degree of TSH of the observation group was less than that of the control group(P<0.05).Afer treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and Creactive protein(CRP)were decreased in both groups,and the reduction range of TNF-α,IL-6 and CRP in observation group was greater than that in control group(P<0.05).The incidence of complications in the observation group(8.57%)was lower than that in the control group(30.30%)(P<0.05).Conclusion:The use of ultrasound-guided microwave radiofrequency ablation in the treatment of thyroid nodules can effectively avoid hypothyroidism,reduce the level of inflammatory factors,and ensure safety,which is worthy of clinical promotion.
