摘要
目的探讨下腔静脉滤器(IVCF)置入联合重组人尿激酶原(rhPro-UK)经导管接触性溶栓(CDT)治疗急性下肢深静脉血栓形成(DVT)的临床效果。方法选取2019年6月至2021年12月本院收治的100例急性下肢DVT患者作为研究对象,随机将其分为对照组与观察组,每组50例。对照组实施rhPro-UK经CDT治疗,观察组实施rhPro-UK经CDT+IVCF置入治疗。比较两组的治疗效果。结果观察组的临床治疗总有效率显著高于对照组(P<0.05)。观察组的治疗时长、住院时间短于对照组,尿激酶原用量、残余狭窄处病灶数量均显著少于对照组(P<0.05)。治疗后,观察组的凝血酶原时间(PT)、活化部分凝血酶时间(APTT)长于对照组,纤维蛋白原(Fig)水平低于对照组(P<0.05)。治疗后,观察组的大腿周径差、小腿周径差、静脉通畅评分均显著低于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论IVCF置入+CDT方案可显著提升急性下肢DVT患者临床治疗效果,有助于降低栓塞、出血风险,缩短治疗时间,具有显著临床应用价值。
Objective To investigate the clinical effect of inferior vena cava filters(IVCF)implantation combined with recombinant human pro-urokinase(rhPro-UK)catheter-directed thrombolysis(CDT)in the treatment of acute lower extremity deep venous thrombosis(DVT).Methods A total of 100 patients with acute lower extremity DVT admitted in our hospital from June 2019 to December 2021 were selected as the research objects,and the patients were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with rhPro-UK by CDT,and the observation group was treated with rhPro-UK by CDT+IVCF implantation.The therapeutic effects of the two groups were compared.Results The total effective rate of clinical treatment in the observation group was significantly higher than that in the control group(P<0.05).The treatment time and hospitalization time of the observation group were shorter than those of the control group,and the dosage of prourokinase and the number of lesions at residual stenosis were significantly less than those of the control group(P<0.05).After treatment,the prothrombin time(PT)and activated partial thromboplastin time(APTT)in the observation group were longer than those in the control group,and the fibrinogen(Fig)level was lower than that in the control group(P<0.05).After treatment,the thigh circumference difference,calf circumference difference and venous patency score of the observation group were significantly lower than those of the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The IVCF implantation+CDT scheme can significantly improve the clinical treatment effect of patients with acute lower extremity DVT,and help to reduce the risk of embolism and bleeding,shorten the treatment time,which has significant clinical application value.
