摘要
目的:研究256排能谱CT血管造影(CTA)在颅内动脉瘤诊治中的应用价值。方法:回顾性收集2020年5月—2022年12月九江学院附属医院神经外科收治的105例疑似颅内动脉瘤患者的病历资料,所有患者均接受256排能谱CTA检查及3D-数字减影血管造影(3D-DSA)检查。以数字减影血管造影(DSA)结果为金标准,诊断为颅内动脉瘤者共63例。评估CTA对颅内动脉瘤的诊断价值,比较确诊患者两种检查的检查时间、检查费用、射线摄入量及不良反应发生情况。结果:经CTA检查,共检出阳性67例,诊断敏感度为95.24%,特异度为83.33%,阳性预测值为89.55%,阴性预测值为92.11%,准确度为90.48%,且一致性较好(Kappa=0.819,P<0.05);256排能谱CTA检查时间及检查费用均低于3D-DSA检查,差异均有统计学意义(P<0.05);3D-DSA检查与256排能谱CTA检查的不良反应总发生率差异无统计学意义(P>0.05)。结论:256排能谱CTA检查颅内动脉瘤患者检查时间等均优于3D-DSA检查,可应用于颅内动脉瘤的筛查及随访复查。
Objective:To study the application value of 256-slice energy spectrum CT angiography(CTA)in the diagnosis and treatment of intracranial aneurysm.Method:The medical records of 105 patients with suspected intracranial aneurysm who were admitted to the Department of Neurosurgery,Jiujiang University Affiliated Hospital from May 2020 to December 2022 were collected retrospectively.All patients were examined with 256-slice energy spectrum CTA and 3D-digital subtraction angiography(3D-DSA).A total of 63 cases were diagnosed as intracranial aneurysms by using digital subtraction angiography(DSA)as the gold standard.The diagnostic value of CTA for intracranial aneurysms was evaluated.Examination time,examination cost,radiation intake and the incidence of adverse reactions were compared between the two examination methods for confirmed patients.Result:After CTA examination,a total of 67 cases were detected as positive,with a diagnostic sensitivity of 95.24%,specificity of 83.33%,positive predictive value of 89.55%,negative predictive value of 92.11%,accuracy of 90.48%,and good consistency(Kappa=0.819,P<0.05).The examination time and cost of 256-slice energy spectrum CTA were lower than those of 3D-DSA,the differences were statistically significant(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between 3D-DSA and 256-slice energy spectrum CTA(P>0.05).Conclusion:The examination time and so on of 256-slice energy spectrum CTA in patients with intracranial aneurysms are better than those of 3D-DSA,and it can be used for the screening and follow-up of intracranial aneurysm.
