摘要
目的 探讨结直肠癌组织中CD146基因甲基化改变与患者临床病理特征和生存的关系。方法 应用甲基化特异性聚合酶链反应(polymerase chain reaction,PCR)检测方法,分析CD146基因甲基化。采用电化学发光法进行定量检测结直肠癌患者血清癌胚抗原(carcinoembryonic antigen,CEA)含量。采用Kaplan-Meier生存曲线计算结直肠癌患者生存率。结果 与结直肠癌组相比,对照组和腺瘤组CD146基因甲基化阳性率差异有统计学意义(P<0.05)。CD146基因甲基化与结直肠癌患者淋巴结转移、浸润深度和TNM分期之间,差异有统计学意义(P<0.05)。CD146基因甲基化与CEA含量之间明显相关(P<0.05)。Kaplan-Meier生存曲线经Log-rank检验发现,CD146基因甲基化与结直肠癌患者总体5年生存率之间存在相关性(P<0.05)。结论 CD146基因表观遗传学的改变参与了结直肠癌的发生、发展。检测CD146基因甲基化可作为反映结直肠癌患者部分恶性生物学行为和生存期的指标。
Objective To investigate the relationship between CD146 gene methylation,clinico-pathological features,and survival of patients with colorectal cancer.Methods The expression of CD146 promoter methylation were detected by methylation-specific polymerase chain reaction.The content of serum carcinoembryonic antigen(CEA) in patients with colorectal cancer was quantitatively detected by electrochemiluminescence.The survival rate of patients with colorectal cancer was evaluated by Kaplan-Meier survival curves.Results Compared with the colorectal cancer group,the methylation positive rate of CD146 gene promoter methylation of control group and adenoma group were significantly different(P<0.05).There were significant differences between CD146 gene promoter methylation and lymph node metastasis,tumor infiltrating depth,TNM stage(P<0.05).There was significant correlation between CD146 gene promoter methylation and CEA content(P<0.05).Kaplan-Meier survival curves tested by Log-rank test showed that there was correlation between CD146 gene promoter methylation and 5-year overall survival rate(P<0.05).Conclusion CD146 gene epigenetic alteration may be related with the occurrence and development of colorectal cancer.The detection for CD146 gene promoter methylation may be helpful to evaluate partial malignant biological behavior and survival of patients with colorectal cancer.
