摘要
目的探讨甲胎蛋白(AFP)、人附睾蛋白4(HE4)、糖类抗原125(CA125)、糖类抗原199(CA199)联合检测在卵巢癌辅助诊断中的应用价值。方法抽取2021年4月至2023年4月新乡市第一人民医院收治的84例卵巢癌患者纳入观察组,将同期81例卵巢良性病变患者纳入对照组,另抽取同期进行体检的90例健康人纳入健康组。检测并比较三组对象血清AFP、HE4、CA125及CA199水平,绘制试者工作特征曲线,分析AFP、HE4、CA125及CA199联合检测对卵巢癌的诊断效能。结果观察组血清AFP为(34.89±5.63)ng/ml、HE4为(201.59±15.34)pmol/L、CA125为(58.97±6.98)U/ml、CA199为(49.74±4.25)U/ml,高于对照组的(14.89±3.01)ng/ml、(103.95±12.63)pmol/L、(21.87±3.49)U/ml、CA199(22.61±3.08)U/ml及健康组的(10.31±2.36)ng/ml、(71.59±8.97)pmol/L、(13.24±2.01)U/ml、(15.75±2.36)U/ml,且对照组各项指标高于健康组(P<0.05)。受试者工作特征曲线结果显示,AFP、HE4、CA125及CA199联合诊断卵巢癌的曲线下面积(AUC)为0.939,高于单项指标诊断(AUC=0.741、0.832、0.837、0.810)。结论AFP、HE4、CA125及CA199在卵巢癌患者机体内呈异常高表达,联合检测能够为卵巢癌的诊断提供一定的参考。
Objective To analyze the application of combined detection ofα-fetoprotein(AFP),human epididymis protein 4(HE4),carbohydrate antigen 125(CA125)and carbohydrate antigen 199(CA199)in the auxiliary diagnosis of ovarian cancer.Methods Eighty-four patients with ovarian cancer admitted to Xinxiang First People’s Hospital from April 2021 to April 2023 were selected and enrolled in the observation group,while 81 patients with benign ovarian lesions were allocated into the control group during the same period,and 90 healthy people who underwent physical examination during the same period were selected into the healthy group.The serum levels of AFP,HE4,CA125 and CA199 were detected and compared among the three groups.The receiver operating characteristic curve was drawn to analyze the efficacy of combined detection of AFP,HE4,CA125 and CA199 in the diagnosis of ovarian cancer.Results In the observation group,the serum AFP level was(34.89±5.63)ng/ml,HE4 level was(201.59±15.34)pmol/L,CA125 level was(58.97±6.98)U/ml,CA199 level was(49.74±4.25)U/ml;and the levels of the four indicators in the observation group were higher than those in the control group,(14.89±3.01)ng/ml,(103.95±12.63)pmol/L,(21.87±3.49)U/ml and(22.61±3.08)U/ml,correspondingly,and were higher than those in the healthy group,(10.31±2.36)ng/ml,(71.59±8.97)pmol/L,(13.24±2.01)U/ml and(15.75±2.36)U/ml,P<0.05.The above indicators of the control group were higher than those of the healthy group(P<0.05).Results of receiver operating characteristic curve showed that the area under the curve(AUC)of combined detection of AFP,HE4,CA125 and CA199 in diagnosing ovarian cancer was 0.939,which was higher than that of the 4 single index diagnoses(AUC=0.741,0.832,0.837,0.810).Conclusions AFP,HE4,CA125 and CA199 are abnormally highly expressed in patients with ovarian cancer.The combined detection of them can provide a certain reference for the diagnosis of ovarian cancer.
