摘要
目的 研究GDM母亲的子代婴儿血清25-羟维生素D、TNF-α与糖脂代谢、胰岛素抵抗的相关性。方法 本研究采用前瞻性研究,2019年3月1日至2020年3月31日在我院生产的GDM母亲的子代婴儿120例作为研究组,另选取同期进行健康体检的120例受试者作为对照组,比较两组受试者的25(OH)D3、TNF-α、胰岛素抵抗以及糖脂代谢之间的相关性,研究25(OH)D3、TNF-α与糖脂代谢以及胰岛素抵抗水平的相关性。结果 研究组患者的25(OH)D3(t=58.886,P<0.001)低于对照组,TNF-α(t=29.224,P<0.001)高于对照组;研究组患者的TG(t=5.746,P<0.001)、TC(t=6.710,P<0.001)、LDL(t=16.287,P<0.001)、FBG(t=2.128,P=0.034)、HbA1c(t=8.664,P<0.001)显著高于对照组,HDL(t=11.239,P<0.001)显著低于对照组,研究组患者的HOMA-IR(t=44.950,P<0.001)以及HOMA-β(t=39.225,P<0.001)水平显著高于对照组;通过相关性分析,25(OH)D3与TG、TC、LDL、FBG、HbA1c、HOMA-IR、HOMA-β呈现负相关,与HDL呈现正相关,TNF-α与TG、TC、LDL、FBG、HbA1c、HOMA-IR、HOMA-β呈现正相关,与HDL呈现负相关。结论 GDM母亲的子代婴儿血清25-羟维生素D、TNF-α与糖脂代谢、胰岛素抵抗呈现显著的相关性,可作为临床诊断的重要依据。
Objective To study the correlation between serum 25-hydroxyvitamin D,TNF-α,glycolipid metabolism and insulin resistance in infants with GDM.Methods In this prospective study,120 cases of GDM mothers and infants who gave birth in our hospital from March 1,2019 to March 31,2020 were selected as the research objects,while 120 cases of healthy patients who underwent physical examination during the same period were selected as the control group.The correlations between 25(OH)D 3,TNF-α,insulin resistance and glucolipid metabolism were evaluated and compared between the two groups.Results 25(OH)D 3(t=58.886,P<0.001)in the study group was lower than that in the control group,while TNF-α(t=29.224,P<0.001)was higher than that in the control group.TG(t=5.746,P<0.001),TC(t=6.710,P<0.001),LDL(t=16.287,P<0.001),FBG(t=2.128,P=0.034),and HbA1c(t=8.664,P<0.001)in the study group were significantly higher than those in the control group.HDL(t=11.239,P<0.001)was significantly lower than that in the control group,while HOMA-IR(t=44.950,P<0.001)and HOMA-β(t=39.225,P<0.001)levels in the study group were significantly higher than those in the control group.Correlation analysis showed that 25(OH)D 3 was negatively correlated with TG,TC,LDL,FBG,HbA1c,HOMA-IR and HOMA-β,and positively correlated with HDL,while TNF-αwas positively correlated with TG,TC,LDL,FBG,HbA1c,HOMA-IR and HOMA-β,but negatively correlated with HDL.Conclusion Serum 25-hydroxyvitamin D and TNF-αof GDM mothers are significantly correlated with glucolipid metabolism and insulin resistance,which could be used as an important reference basis for the clinical diagnosis.
