摘要
目的分析长链非编码核糖核酸(lncRNA)NORAD、微小核糖核酸(miR)-26b在急性冠状动脉综合征(ACS)患者血清中的表达相关性及二者对ACS的诊断价值。方法选取永州市中心医院2020年1月至2021年12月期间诊治的68例ACS患者纳入ACS组,按照Gensini评分将其分为高水平组、中等水平组、低水平组;同期选基线资料无显著性差异的健康体检者72例为对照组。收集所有研究对象一般资料及相关生化指标水平;采用实时荧光定量PCR(qRT-PCR)法检测血清lncRNA NORAD、miR-26b水平;采用Pearson相关分析评估ACS患者血清lncRNA NORAD、miR-26b水平与一般资料及相关生化指标的相关性;血清lncRNA NORAD、miR-26b检测水平对ACS的诊断效能采用受试者工作特征(ROC)曲线评价;ACS发生的影响因素采用Logistic回归分析。结果ACS组血清lncRNA NORAD水平显著高于对照组,miR-26b水平显著低于对照组(t=10.177、11.175,P<0.05)。高水平组lncRNA NORAD、心肌肌钙蛋白I(cTnI)、脑利钠肽(BNP)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)水平显著高于中等水平组和低水平组,miR-26b水平显著低于中等水平组和低水平组(F=10.227、37.655、23.000、76.319、95.680、107.706、9.763,P<0.05)。ACS患者血清lncRNA NORAD与miR-26b水平呈负相关(r=-0.558,P<0.05);cTnI、BNP、hs-CRP、TNF-α、IL-6与lncRNA NORAD呈正相关,与miR-26b呈负相关(r=0.502、0.510、0.494、0.479、0.486、-0.487、-0.504、-0.493、-0.472、-0.482,P<0.05)。血清lncRNA NORAD、miR-26b及其联合诊断ACS的曲线下面积分别为0.764、0.907、0.914,特异度分别为81.5%、72.2%、93.5%,敏感度分别为72.4%、87.9%、67.8%。miR-26b是ACS发生的保护因素(P<0.05),lncRNA NORAD是ACS发生的独立危险因素(P<0.05)。结论ACS患者血清中lncRNA NORAD、miR-26b表达呈负相关,且二者在ACS的临床诊断中均具有一定价值。
Objective To analyze the correlation between the expression of long non-coding RNA NORAD(lncRNA NORAD)and microRNA-26b(miR-26b)in the serum of patients with acute coronary syndrome(ACS)and their diagnostic value for ACS.Methods A total of 68 ACS patients diagnosed and treated in Yongzhou Central Hospital from January 2020 to December 2021 were included in the ACS group,and classified into high-level group,moderate-level group and low-level group according to Gensini score.A total of 72 healthy individuals with no significant baseline differences during the same period were selected as the control group.General information and biochemical indexes of all study subjects were collected.Serum lncRNA NORAD and miR-26b levels were detected by quantitative real-time PCR(qRT-PCR).Pearson correlation analysis was used to evaluate the correlation between serum lncRNA NORAD and miR-26b levels and general data and biochemical indexes in ACS patients.The diagnostic efficacy of serum lncRNA NORAD and miR-26b levels in ACS was evaluated by receiver operator characteristic(ROC)curve,and the influencing factors of ACS were analyzed by Logistic regression.Results The level of serum lncRNA NORAD in the ACS group was significantly higher than that in the control group,and the level of miR-26b was significantly lower than that in the control group(t=10.177,11.175;P<0.05).The levels of lncRNA NORAD,cardiac troponin I(cTnI),brain natriuretic peptide(BNP),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factorα(TNF-α),and interleukin-6(IL-6)in the high-level group were significantly higher than those in the moderate-level group and low-level group,and the level of miR-26b in the high-level group was significantly lower than that in the moderate-level group and low-level group(F=10.227,37.655,23.000,76.319,95.680,107.706,9.763;P<0.05).There was a negative correlation between the levels of serum lncRNA NORAD and miR-26b in ACS patients(r=-0.558,P<0.05).The levels of cTnI,BNP,hs-CRP,TNF-αand IL-6 were positively correlated with
