摘要
目的 探讨美兰染色法在精准切除乳头溢液病变乳管中的临床应用价值。方法 选取60例确诊为乳头溢液的女性患者进行对照研究,按照手术方式分为研究组和对照组,每组30例。研究组使用美兰染色法精准切除的方式,对照组采用传统切除方式。随访12个月,两组患者分别在术后3、12个月进行乳腺超声检查,观察手术切除面的愈合情况和乳腺内部病变的复发情况,比较两组患者手术时间、术中出血量和术后并发症发生率。结果 术后3、12个月,研究组手术切除面愈合情况优于对照组,乳腺内部病变复发率低于对照组,差异具有统计学意义(P<0.05)。研究组手术时间为(37.23±5.36)min,短于对照组的(48.07±7.22)min,差异有统计学意义(P<0.05)。研究组术中出血量为(22.67±4.12)ml,少于对照组的(29.11±7.87)ml,差异有统计学意义(P<0.05)。研究组术后出现1例并发症,对照组术后出现6例并发症,研究组术后并发症发生率3.33%低于对照组的20.00%,差异有统计学意义(P<0.05)。结论 美兰染色法能够明显提高乳头溢液病变乳管的切除率和术后康复效果,且手术时间、术中出血量和术后并发症发生率均优于传统切除方式,值得在乳腺外科手术中推广应用。
Objective To discuss the clinical value of methylene blue staining in precise excision of diseased breast duct causing nipple discharge.Methods A total of 60 female patients diagnosed with nipple discharge were divided into study group and control group according to different surgical method,with 30 cases in each group.The study group used methylene blue staining for precise excision,while the control group used conventional excision.All patients were followed-up for 12 months,breast ultrasound examination was performed in both groups at 3 months and 12 months after surgery to observe the healing of the surgical resection surface and the recurrence of internal breast lesions,and compared the operative time,intraoperative blood loss and postoperative complications between the two groups.Results At 3 and 12 months after surgery,the healing of the surgical resection surface of the study group was better than that of the control group,and the recurrence rate of internal breast lesions was lower than that of the control group.The differences were statistically significant(P<0.05).The operative time of(37.23±5.36)min in the study group was shorter than that of(48.07±7.22)min in the control group,and the difference was statistically significant(P<0.05).The intraoperative blood loss of(22.67±4.12)ml in the study group was less than that of(29.11±7.87)ml in the control group,and the difference was statistically significant(P<0.05).Postoperatively,there were 1 case of complications in the study group and 6 cases of complications in the control group;the postoperative complication rate of 3.33%in the study group was lower than that of 20.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Methylene blue staining can significantly improve the resection rate and postoperative rehabilitation effect of diseased breast duct causing nipple discharge,and the operative time,intraoperative blood loss and postoperative complication rate are superior to conventional resection methods,which i
