摘要
机械通气患者中约有25%会出现撤机困难或延迟撤机,导致住院时间延长,病情加重甚至死亡。膈肌是最主要的呼吸肌,在自发呼吸中起重要作用,机械通气患者极易出现膈肌功能障碍,造成呼吸肌无力,是呼吸机撤机失败的重要原因。跨膈压作为膈肌功能评估金标准因有创、技术要求高的缺点难以广泛使用,而超声具有即时、无创的优点,可对膈肌结构和功能进行评估,临床中已开始采用膈肌位移、膈肌增厚率等方法预测撤机结局,取得一定成果。文章对近年来膈肌超声在预测撤机结局的应用和进展进行综述。
About 25%of patients with mechanical ventilation experience difficulty in weaning,worsening of the condition,and even death.Diaphragm plays an important role in spontaneous breathing,mechanical ventilation patients prone to diaphragm dysfunction,resulting in respiratory muscle weakness,which is an important reason for the failure of ventilator weaning.Transdiaphragmatic pressure is invasive and has high technical requirements,so it is difficult to be wildly used.Ultrasound has the advantages of instant and non-invasive,which can evaluate the structure and function of diaphragm.In clinical practice,diaphragmatic excursion and diaphragm thickening fraction have been used to predict the outcome of weaning successfully.This review focuses on the application and progress in predicting weaning outcomes by diaphragmatic ultrasound.
