中高通量透析对维持性血液透析患者糖脂代谢的影响 被引量:1

Effect of medium-to-high throughput dialysis on glucose and lipid metabolism in maintenance hemodialysis patients:A randomized controlled study

摘要 目的探究中高通量透析对维持性血液透析患者糖脂代谢的影响。方法选择2020年4月至2022年5月收治的86例尿毒症维持性血液透析患者作为研究对象,采用随机数字表法分为对照组和观察组,每组43例。对照组给予中通量透析,观察组给予高通量,对比2组的临床疗效以及治疗前、治疗3个月的肾功能、代谢情况。结果观察组的临床总有效率(88.37%)高于对照组(69.77%),差异有统计学意义(P<0.05)。治疗3个月,观察组的Scr(t=2.808)、BUN(t=2.433)、CysC(t=3.374)、β2-MG(t=2.378)水平均低于对照组(P<0.05)。治疗3个月,观察组的血钙高于对照组(t=2.280),其血磷(t=2.403)、PTH(t=2.838)均低于对照组(P<0.05)。治疗后,2组FBG(t=2.349)、FINS(t=3.190)、TC(t=2.702)、TG(t=2.926)水平均低于治疗前,且观察组均低于对照组,差异有统计学意义(P<0.05)。结论高通量透析应用于维持性血液透析患者中,能有效提高临床疗效,改善患者的肾功能和代谢情况。 Objective To explore the effect of medium-to-high throughput dialysis on glucose and lipid metabolism in maintenance hemodialysis patients.Methods A total of 86 maintenance hemodialysis patients with uremia admitted to our hospital from April 2020 to May 2022 were selected as the research objects.They were randomly divided into control group and observation group,with 43 cases per group.Patients in the control group were given medium flux dialysis,and those in the observation group were given high flux dialysis.The clinical efficacy,renal function and metabolism before and 3 months after treatment of the two groups were compared.Results The total clinical effective rate in the observation group was significantly higher than that in the control group(88.37%vs 69.77%,P<0.05).After 3 months of treatment,serum creatinine(Scr,t=2.808),blood urea nitrogen(BUN,t=2.433),cystatin C(CysC,t=3.374)andβ2 microglobulin(β2-MG,t=2.378)in the observation group were significantly lower than those in the control group(P<0.05).After 3 months of treatment,the serum calcium in the observation group was significantly higher than that in the control group(t=2.280),and serum phosphorus(t=2.403)and parathyroid hormone(PTH,t=2.838)were significantly lower(P<0.05).After treatment,fasting blood glucose(t=2.349),fasting insulin(FINS,t=3.190),total cholesterol(TC,t=2.702),and triglyceride(TG,t=2.926)in both groups were significantly lower than those before treatment,which were significantly lower in the observation group than those in the control group(P<0.05).Conclusion The application of high-throughput dialysis in maintenance hemodialysis patients can effectively improve the clinical efficacy,renal function and metabolism.
