摘要
目的:观察富含血小板凝胶联合生肌玉红膏治疗糖尿病足创面的临床疗效。方法:选取平顶山市中医医院门诊及住院治疗的符合纳入标准的60例糖尿病足患者,按照随机数字表法随机分为两组,对照组30例,治疗组30例。治疗组给予富含血小板凝胶联合生肌玉红膏外用治疗,对照组给予生肌玉红膏外用治疗。两组均治疗2个疗程(14 d为一个疗程)。统计分析对照组和治疗组患者治疗前及治疗2个疗程结束后血清中炎性指标[白细胞计数(White Blood Cell Count,WBC)、C反应蛋白(C-reactive Protein,CRP)]、创面愈合有效率、创面面积变化及创面各症状体征积分变化等。结果:对照组中创面愈合总有效率为90.00%(27/30),治疗组中创面愈合总有效率为96.67%(29/30),治疗组创面愈合疗效优于对照组(P<0.05)。治疗2个疗程结束后两组糖尿病足患者创面面积均下降,治疗组创面面积情况优于对照组(P<0.05);两组血清中炎性指标(WBC、CRP)均降低,治疗组炎性指标降低程度优于对照组(P<0.05);两组创面各症状体征积分均下降(P<0.05),且治疗组优于对照组(P<0.05)。结论:富含血小板凝胶联合生肌玉红膏治疗糖尿病足创面效果显著,能明显改善患者临床症状,改善患者创面感染情况,减少创面的渗出,增加创面肉芽组织生长,提高创面愈合率,减轻患者及患者家属的经济及心理负担。
Objective:To observe the clinical effect of platelet-rich gel combined with Shengji Yuhong ointment(生肌玉红膏)in the treatment of diabetic foot wounds.Methods:A total of 60 patients with diabetic foot in Outpatient Department and Inpatient Department in Pingdingshan Hospital of Traditional Chinese Medicine who met the inclusion criteria were selected and randomly divided into two groups according to random number table method,with 30 cases in the control group and 30 cases in the treatment group.The treatment group was given plateletrich gel combined with Shengji Yuhong ointment for external treatment,and the control group was given Shengji Yuhong ointment for external treatment.Both groups were treated for 2 courses(14 days was for 1 course).Serum inflammatory indexes(WBC,CRP),wound healing efficiency,wound area change,and wound symptom and sign score change were statistically analyzed in the control group and the treatment group before treatment and after 2 courses of treatment.Results:The total effective rate of wound healing in the control group was 90.00%(27/30),and that in the treatment group was 96.67%(29/30).The curative effect of wound healing in the treatment group was better than that in the control group(P<0.05).After 2 courses of treatment,the wound area of diabetic foot patients in both groups were decreased,and the healing of the wound area in the treatment group was better than that in the control group(P<0.05).Serum inflammatory indexes(WBC,CRP)in both groups were decreased,and inflammatory indexes in the treatment group were better than those in the control group(P<0.05).The scores of wound signs and symptoms in both groups were decreased(P<0.05),and the treatment group was better than the control group(P<0.05).Conclusion:Platelet-rich gel combined with Shengji Yuhong ointment has significant effect in the treatment of diabetic foot wound,which can significantly relieve the clinical symptoms of patients,improve the infection of patients,reduce the exudation of the wound,increase the growth
