改良取材涂片法镜检对足癣真菌阳性率的影响

摘要 目的:分析改良取材涂片法镜检对足癣真菌阳性的检出情况。方法:前瞻性随机对照研究,选取新余银河医院2020年10月—2022年9月收治的100例高度疑似足癣的患者作为研究对象,采用随机数字表法将患者分为试验组与对照组,各50例。两组患者均接受真菌培养检测,试验组采用改良取材涂片法镜检,对照组采用常规取材涂片直接镜检法。比较两组患者取材时的疼痛程度,另以真菌培养结果为诊断金标准,使用Kappa一致性检验改良取材涂片法镜检、常规取材涂片直接镜检法与真菌培养结果的一致性,比较二者诊断足癣的效能。结果:试验组、对照组真菌培养阳性均为46例,阴性均为4例。试验组经改良取材涂片法镜检检出阳性45例,阴性5例;对照组经常规取材涂片直接镜检法检出阳性38例,阴性12例。Kappa一致性检验显示,改良取材涂片法镜检与真菌培养检出结果具有较强一致性(Kappa=0.634,P<0.001);常规取材涂片直接镜检法与真菌培养检出结果具有一般一致性(Kappa=0.290,P<0.05)。改良取材涂片法镜检的敏感度、准确度均高于常规取材涂片直接镜检法,差异均有统计学意义(P<0.05),两者特异度比较,差异无统计学意义(P>0.05)。试验组患者的视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05)。结论:相较于常规取材涂片直接镜检法,改良取材涂片法镜检对真菌具有更高的检出率,且可减轻患者取材时的疼痛程度。 Objective:To analyze the the positive detection of tinea pedis fungus by modified sampling smear method.Method:Prospective randomized controlled study.A total of 100 patients with high suspected tinea pedis admitted to Xinyu Yinhe Hospital from October 2020 to September 2022 were selected as the study objects.The patients were divided into experimental group and control group by random number table method,with 50 cases in each group.The two groups of patients were tested by fungal culture,the experimental group was tested by modified sampling smear method microscopic examination,and the control group was tested by routine sampling smear method direct microscopic examination.The pain degree of the two groups was compared during sampling,and the result of fungal culture was taken as the gold standard for diagnosis.Kappa consistency check was used to test the consistency of the results of modified sampling smear method microscopic examination,routine sampling smear method direct microscopic examination and fungal culture,and the efficacy of the two methods in diagnosing tinea plantar was compared.Result:In both experimental group and control group,46 cases were positive and 4 cases were negative.In the experimental group,45 cases were positive and 5 cases were negative by the modified sampling smear method microscopic examination.In the control group,38 cases were positive and 12 cases were negative by routine sampling smear method direct microscopic examination.Kappa consistency check showed that there was a strong consistency between the result of modified sampling smear method microscopic examination and fungal culture(Kappa=0.634,P<0.001),and there was a generally consistency between the result of routine sampling smear method direct microscopic examination and fungal culture(Kappa=0.290,P<0.05).The sensitivity and accuracy of the modified sampling smear method microscopic examination were higher than those of the routine sampling smear method direct microscopic examination,the difference was statistically signi
