摘要
目的比较7野固定野调强(IMRT)与容积旋转调强(VMAT)两种放射治疗技术在前列腺癌放疗计划独立验算中的剂量差异,并检查±5%的验算差异作为IMRT/VMAT计划执行阈值的合理性。方法使用基于蒙特卡罗的商业验证软件VERIQA对22例前列腺癌的IMRT/VMAT计划进行验算分析。危及器官限量参考临床常规要求,靶区接受标准为100%处方剂量要包饶95%计划靶区(PTV)体积。使用均值±标准差比较计划系统和验证软件间的剂量差异。结果在前列腺癌靶区的剂量验算比较中,7IMRT与VMAT计划间差异无统计学意义(P>0.05)。在危及器官的比较中,7IMRT计划中膀胱的平均、中位、最大剂量以及直肠的最小、平均、中位剂量的验算差异较VMAT计划更大,差异有统计学意义(P<0.05)。结论前列腺癌计划中7IMRT与VMAT关于靶区的剂量验算差异具有一致性,局部剂量差异在±1%以内。对于前列腺IMRT/VMAT计划,±1%的验算差异或可作为调强计划执行阈值。
【Objective】To compare the dose difference between 7-field fixed-field intensity-modulated radiotherapy(IMRT)and volumetric modulated arc therapy(VMAT)in the independent check of prostate cancer radiotherapy plan,and check the rationality of using±5%check difference as the implementation threshold of IMRT/VMAT plan.【Methods】The IMRT/VMAT plans of 22 prostate cancer patients were checked and analyzed using the commercial verification software VERIQA based on Monte Carlo.Refer to the clinical routine requirements for the limit of organs at risk.Both plans satisfied the condition of 95%PTV covered by 100%prescription.The doses from the treatment planning system(TPS)and the validation software were compared using the mean±standard deviation.【Results】There was no statistical difference in the dose difference of PTV between the 7IMRT and VMAT plans.The average,median,maximum dose of bladder,and the minimum,average,median dose of rectum for 7IMRT plan were higher than those for the VMAT plan,and the differences had statistical significance.【Conclusion】The dose difference of PTV between 7IMRT and VMAT in the prostate cancer plan is consistent,and the local dose difference is within±1%.For the prostate IMRT/VMAT plan,the difference of±1%may be used as the implementation threshold of the intensity-modulated radiotherapy plan.
