基于能谱CT单能量技术对颈动脉粥样硬化图像质量的研究

摘要 目的:探究能谱CT单能量成像技术对观察颈动脉粥样硬化图像质量的影响,评估最佳单能量水平。方法:回顾性分析本院收治的80例颈动脉粥样硬化患者的资料,均行颈动脉能谱CTA检查,经后处理获得单能量图像(40~140 keV,间隔10 keV)和常规120 kVp-like图像。测量并比较40~70 keV单能量图像和常规组的图像噪声,主动脉弓、双侧颈总动脉、双侧颈内动脉及非钙化斑块和钙化斑块的对比噪声比(Contrast to noise ratio,CNR),同时应用5分法对图像行主观评价;测量斑块短径和狭窄程度,对结果进行统计学分析。结果:随着能级减小,图像噪声、各支血管和斑块的CNR均呈增高趋势,在40 keV时达到最高。各支血管和斑块的CNR在40~70 keV高于常规120 kVp-like组,其中主动脉弓、颈总动脉和非钙化斑块的CNR在40~60 keV水平与常规120 kVp-like组比较有统计学意义,颈内动脉和钙化斑块则在40~50 keV水平(P均<0.05)。40~70 keV单能量图像主观评分均优于常规组,以60 keV单能量图像最佳(P<0.001)。低keV图像产生明显更高的钙化斑块短径/狭窄程度和更小的非钙化斑块短径/狭窄程度(P均<0.001)。结论:与120 kVp-like图像相比,低keV的单能量图像可获得更好的图像质量,但可导致颈动脉斑块大小及狭窄程度的改变,60 keV为显示颈部动脉粥样硬化的最佳单能级。 Objective:To explore the effect of spectral CT monochromatic image technique on the image quality of carotid atherosclerosis,and to evaluate the optimal monochromatic energy level.Methods:The data of 80 patients with carotid atherosclerosis in our hospital were retrospectively analyzed.All patients were examined by neck CT angiography on the spectral CT.Monochromatic energy images(40~140 keV,with 10 keV interval)and conventional 120 kVp-like images were obtained after post-processing.The image noise and contrast to noise ratio(CNR)of aortic arch,common carotid arteries,internal carotid arteries,non-calcified plaques and calcified plaques were measured and compared on 40~70 keV and conventional 120 kVplike images.Then the 5-point scoring method was applied for subjective evaluation.The short diameter and luminal narrowing of plaque were measured and the results were statistically analyzed.Results:With the decrease of energy level,the image noise and CNR of carotid artery and plaques showed a trend of increasing,and achieved the highest value at 40 keV.The CNR of each artery and plaque at the 40~70 keV monochromatic energy images was higher than conventional 120 kVp-like images.The CNR of aortic arch,common carotid artery and non-calcified plaque was statistically significant at the 40~60 keV monochromatic energy images compared with conventional 120 kVp-like images.The CNR of internal carotid artery and calcified plaque was at the 40~50 keV(both P<0.05).The subjective scores of 40~70 keV monochromatic energy images were better than those of conventional images,then the 60 keV monochromatic energy images demonstrated the best subjective scores(both P<0.001).In addition,low keV images yielded higher short diameters/stenosis of calcified plaques and smaller short diameters/stenosis of non-calcified plaque(both P<0.001).Conclusion:Compared with 120 kVp-like images,low energy images can obtain better image quality,but can result in differences in sizes of carotid plaques and the degree of stenosis.The optimal monochrom
