摘要
目的分析加味四黄汤内服外敷用于混合痔吻合器痔上黏膜环切术(PPH)术后对患者疼痛应激、血清白介素1(IL-1)、C-反应蛋白(CRP)、前列腺素2E(PGE-2)水平的影响。方法选取2018年2月~2020年9月在我院接受PPH术治疗治疗的气滞血瘀型混合痔患者150例,按照随机数字表法划分为对照组和观察组,每组75例。对照组患者术后采用常规西医和高锰酸钾溶液坐浴治疗,观察组在常规西医治疗基础上给予加味四黄汤内服外敷治疗,两组术后连续治疗14d,比较两组混合痔PPH术后治疗效果,治疗前后疼痛指数、疼痛时间和创面愈合时间,比较治疗前后血清IL-1、GRP和PGE-2水平以及两组患者的并发症。结果观察组混合痔PPH术后肛门疼痛患者的治疗有效率高于对照组(P<0.05);观察组肛门疼痛指数、肛门疼痛时间和创面愈合时间均明显低于对照组(P<0.05);治疗后,观察组患者血清IL-1、GRP和PGE-2水平明显低于对照组,且观察组下降幅度大于对照组(P<0.05);观察组术后肛周血肿和排尿困难发生率明显低于对照组(P<0.05)。结论加味四黄汤内服外敷可以降低IL-1、GRP和PGE-2水平,减轻混合痔PPH术后患者疼痛症状,降低术后并发症的发生。
Objective To analyze the effects of oral and external application of modified Sihuang Decoction on pain stress,se-rum interleukin-1(IL-1),C-reactive protein(CRP)and prostaglandin 2E(PGE-2)levels in patients with mixed hemor-rhoids after procedure for prolapse and hemorrhoids(PPH).Methods A total of 15Opatients with mixed hemorrhoids of qi-stagna-tion and blood stasis type undergoing PPH in the hospital were enrolled between February 2018and September 2020.According to random number table method,they were divided into control group and observation group,75cases in each group.After surgery,control group was treated with routine westerm medicine and potassium permanganate solution for sitz bath,while observation group was given oral and external application of modified Sihuang Decoction on basis of routine western medicine.Both groups were con-tinuously treated for 14d.The curative effect,pain index pain time,wound healing time,levels of serum IL-1,CRP and PCE-2before and after treatment,and complications in the two groups were compared.Results The response rate of treatment in observation group was higher than that in control group(P<0.05).The anal pain index,anal pain time and wound healing time in observation group were significantly lower than those in control group(P<0.05).After treatment,levels of serum IL-1,CRP and PGE-2in observation group were significantly lower than those in control group,and the decline amplitude was greater in observation group than control group(P<0.05).The incidence of postoperative perianal hematoma and dysuria in observation group was significant lower than that in control group(P<0.05).Conclusion Oral and external application of modified Sihuang Decoction can reduce levels of IL-1,CRP and PCE-2,alleviate pain symptoms of patients with mixed hemorrhoids after PPH,and reduce the occurrence of postoperative complications.
