摘要
目的探讨miR-29、miR-21和miR-133在持续心房颤动合并射血分数减低患者中的表达及临床意义。方法选取2020年1月至2022年4月持续性心房颤动合并射血分数减低患者88例(A组),持续性心房颤动合并射血分数保留患者80例(B组),同时健康志愿者80例(C组)作为对照,检测各组血清miR-29、miR-21和miR-133表达;同时检测A组和B组超声心动图参数,分析miR-29、miR-21和miR-133与超声心动图、患者预后的关系。结果A组血清miR-29相对表达量为0.66±0.18,明显低于B组和C组(P<0.05),而miR-21和miR-133相对表达量分别为1.64±0.19和1.82±0.21,明显高于B组和C组(P<0.05);B组血清miR-29相对表达量为0.80±0.13,明显低于C组(P<0.05),而miR-21和miR-133相对表达量分别为1.35±0.20和1.55±0.24,明显高于C组(P<0.05)。A组左心房直径(LAD)、左心室舒张期直径(LVDD)和左心室收缩末期直径(LVSD)分别为(46.60±3.74)mm、(65.50±3.19)mm和(52.28±3.08)mm,明显高于B组(P<0.05),而左心室射血分数(LVEF)为(32.29±3.20)%,明显低于B组(P<0.05)。A组患者miR-29与LAD、LVDD、LVSD呈负相关(r=-0.434、-0.384、-0.334,P<0.05),而与LVEF呈正相关(r=0.476,P<0.05);miR-21、miR-133与LAD呈正相关(r=0.451、0.428,P<0.05),而与LVEF呈负相关(r=-0.501、-0.487,P<0.05)。A组预后不良患者血清miR-29相对表达量为0.59±0.16,明显低于预后良好患者(P<0.05),而miR-21和miR-133相对表达量分别为1.73±0.22和1.90±0.23,明显高于预后良好患者(P<0.05)。血清miR-29、miR-21和miR-133预测预后不良的受试者工作特征(ROC)曲线下面积分别为0.762、0.674和0.680(P<0.05)。结论持续心房颤动合并射血分数减低患者血清miR-29表达降低,而miR-21和miR-133表达升高,三者表达与患者超声心动图参数及预后有关,在预测患者预后方面有一定应用价值。
Objective To investigate the expression and clinical significance of miR-29,miR-21 and miR-133 in patients with persistent atrial fibrillation and reduced ejection fraction.Methods From January 2020 to April 2022,88 patients with persistent atrial fibrillation and decreased ejection fraction(Group A),80 patients with persistent atrial fibrillation and preserved ejection fraction(Group B),and 80 healthy volunteers(Group C)as controls were selected.The expression of miR-29,miR-21,and miR-133 in serum of each group was detected.At the same time,the echocardiographic parameters of Group A and Group B were detected,and the correlation between miR-29,miR-21,and miR-133 and echocardiogram and the prognosis of patients was analyzed.Results The relative expression of miR-29 in Group A was 0.66±0.18,which was significantly lower than those in Group B and Group C(P<0.05);while the relative expression of miR-21 and miR-133 were 1.64±0.19 and 1.82±0.21,respectively,which were significantly higher than those in Group B and Group C(P<0.05).The relative expression of miR-29 in serum of Group B was 0.80±0.13,which was significantly lower than that in Group C(P<0.05);while the relative expression of miR-21 and miR-133 were 1.35±0.20 and 1.55±0.24,respectively,which were significantly higher than that in Group C(P<0.05).The left atrial diameter(LAD),left ventricular diastolic diameter(LVDD)and left ventricular end systolic diameter(LVSD)in Group A were(46.60±3.74)mm,(65.50±3.19)mm and(52.28±3.08)mm,respectively,which were significantly higher than those in Group B(P<0.05);while the left ventricular ejection fraction(LVEF)was(32.29±3.20)%,which was significantly lower than that in Group B(P<0.05).In Group A,miR-29 was negatively correlated with LAD,LVDD,and LVSD(r=-0.434,-0.384,-0.334,P<0.05);but positively correlated with LVEF(r=0.476,P<0.05);miR-21 and miR-133 were positively correlated with LAD(r=0.451,0.428,P<0.05),but negatively correlated with LVEF(r=-0.501,-0.487,P<0.05).The relative expression of serum miR-29 in pa
