摘要
目的分析256层螺旋CT左房肺静脉成像扫描在心房颤动患者左心耳径线与容积定量的特征及临床意义。方法本研究属双盲法,选取2020年6月至2022年6月郑州市第七人民医院心血管内科收治的非瓣膜性心房颤动患者82例为本次研究的观察组,并同期选取非心房颤动者50例为对照组。两组均接受256层螺旋CT检查,采用左房肺静脉成像扫描检查方案,所得图像均采用图像后处理技术对患者左心耳进行模拟重建,测量左心耳长径、短径、面积、周长、体积及左心房体积。结果两组性别、年龄、身高、体重、体重指数等一般资料对比,差异无统计学意义(P>0.05),但就数据而言男性心房颤动者多于女性。采用256层螺旋CT客观研究两组左心房结构、容积、左心耳解剖径线以及不同性别左心耳解剖径线、不同类型的左心房结构及容积指标,结果显示观察组左心房最大容积(LAV_(max))、左心房最小容积(LAV_(min))、左心房最大前后径(LAD_(max))、左心房最小前后径(LAD_(min))、左心耳容积(LAAV)数据均大于对照组(P<0.05);观察组左心耳开口短径大于对照组(P<0.05);但观察组左心耳开口长径、面积、周长、深径与对照组相比差异均无统计学意义(P>0.05);不同性别左心耳开口长径、短径、面积、周长、深径、LAAV对比差异均无统计学差异(P>0.05);因男性身高、体重存在差异,本次研究对男女身高、体重进行体表面积标化后进一步比较,结果显示女性左心耳开口长径、短径、面积、周长、深径、LAAV均大于男性(P<0.05);持续性心房颤动患者LAD_(max)、LAD_(min)、LAV_(max)、LAV_(min)、LAAV均高于阵发性心房颤动患者(P<0.05)。结论多层螺旋CT成像下心房颤动患者左心耳体积增大,与性别无关,与心房颤动类型有关,且左心耳解剖径线与性别存在相关性,对不同类型心房颤动患者左心房结构改变及左心耳血栓形成具有指导�
Objective To analyze the characteristics and clinical significance of 256-slice spiral CT imaging of left atrial pulmonary vein in the quantification of the diameter and volume of the left atrial ear in patients with atrial fibrillation.Methods This study was double-blind.A total of 82 patients with non-valvular atrial fibrillation admitted to the cardiovascular department of Zhengzhou Seventh People’s Hospital from June 2020 to June 2022 were selected as the observation group,and 50 patients with non-atrial fibrillation were selected as the control group during the same period.All patients underwent 256-slice spiral CT examination.Pulmonary vein scanning was used in atrial fibrillation group,and left atrial pulmonary vein imaging scanning was used in non-atrial fibrillation patients.Image post-processing technology was used to simulate reconstruction of the left atrial ear of patients,and long diameter,short diameter,area,circumference,volume and left atrial volume of the left atrial ear were measured in all the images obtained.Results There were no significant differences in gender,age,height,weight,body mass index and other baseline data between the two groups(P>0.05),but in terms of data,there were more atrial fibrillation patients in males than females.256-Slice spiral CT was used to objectively study the left atrial structure,volume,anatomic diameter of the left atrial appendage of the two groups,as well as the anatomic diameter of the left atrial appendage of different genders,and the structure and volume indexes of different types of the left atrial appendage.The results showed that the data of left atrial maximum volume(LAV_(max)),left atrial minimum volume(LAV_(min)),left atrial maximum anterioranterior diameter(LAD_(max)),left atrial minimum anterioranterior diameter(LAD_(min))and left atrial auricular volume(LAAV)in the observation group were greater than those in the control group,and the short diameter of left auricular opening in the observation group was larger than that in the control group(P<0
