摘要
目的分析多囊卵巢综合征(PCOS)患者中血清脂联素(APN)、瘦素(LP)水平与胰岛素抵抗(IR)的相关性。方法选取73例PCOS患者作为观察组,另选取63例月经周期正常、排卵正常、因男方因素或输卵管因素不孕而就诊的育龄患者作为对照组。对所有患者的APN、LP、空腹血糖(FPG)、空腹胰岛素(FINS)指标进行检测,并计算稳态模型胰岛素抵抗指数(HOMA-IR)。观察并比较其APN、LP水平与HOMA-IR的相关性。结果观察组FINS、FPG、HOMA-IR均显著高于对照组,差异均有统计学意义(P<0.05);观察组APN表达水平显著低于对照组,LP水平显著高于对照组,差异均有统计学意义(P<0.05)。Spearman相关分析结果显示,PCOS患者的血清LP与HOMA-IR成正相关(r=0.890,P<0.01)。APN与HOMA-IR成负相关(r=-0.484,P<0.01),LP与APN表达水平成负相关(r=-0.544,P<0.01)。结论PCOS患者均存在APN、LP水平异常,LP水平升高、APN表达水平降低均可导致患者出现胰岛素抵抗,三者之间存在着明显的相关性。
Objective To analyze the correlation between serum adiponectin(APN),leptin(LP)levels and insulin resistance(IR)in patients with polycystic ovary syndrome(PCOS).Methods 73 patients with PCOS were selected as the observation group,and another 63 patients of childbearing age with normal menstrual cycle,normal ovulation,and infertility due to male or tubal factors were selected as the control group.The APN,LP,fasting plasma glucose(FPG),fasting insulin(FINS)were measured for all personnel,and the insulin resistance index homeostasis model assessment for insulin resistance(HOMA-IR)was calculated.The correlation between their APN and LP levels and HOMA-IR were observed and compared.Results The FINS,FPG and HOMA-IR of the observation group were significantly higher than those of the control group,the differences were statistically significant(P<0.05).The APN expression level of the observation group was significantly lower than that of the control group,and the LP level was significantly higher than that of the control group,the differences were statistically significant(P<0.05).HOMA-IR in PCOS patients was positively correlated with LP(r=0.890,P<0.01),negatively with APN expression level(r=0.484,P<0.01)and LP with APN expression level(r=-0.544,P<0.01).Conclusion Most PCOS patients have abnormal APN and LP levels,and increased LP level and decreased APN expression level can lead to insulin resistance,and there is an obvious correlation among the three.
