摘要
腰椎间盘突出症是以腰腿痛、下肢麻木及神经功能障碍等为主要症状的临床综合征,往往反复发作难愈,中医辨证论治病机复杂,单一治法取效不易。文章基于《临证指南医案》总结叶天士奇经辨证用药规律,阐述腰椎间盘突出症奇经之病机为“督脉阳虚、带脉络瘀”,以“分虚实、通脉络、补肝肾”为主要治则。临证中运用奇经辨证治疗病程长、难治愈的腰椎间盘突出症,可起到“益气血、固脏腑、调正经”的作用,取得较好的疗效。
Lumbar intervertebral disc herniation is a clinical syndrome with lumbar and leg pain,lower limb numbness and neurological dysfunction as the main symptoms,which is often recurrent and difficult to cure,and the pathogenesis of traditional Chinese medicine is complicated,so it is not easy to obtain the effect of a single treatment.This paper summarises YE Tianshi's qi meridian diagnosis and medication rules based on the Linzheng Zhinan Yian,and explains that the pathogenesis of lumbar intervertebral disc herniation disease in Qi meridian is‘deficiency of yang in directing chakra,and stasis in belt chakra and collaterals’and that‘separating the deficiency from the real,clearing the chakra and collaterals and tonifying the liver and kidneys’is the main principle of treatment.In the clinical evidence,the use of qi meridian in the treatment of lumbar intervertebral disc herniation,which is long and difficult to cure,can play the role of‘benefiting qi and blood,fixing the internal organs,and regulating the proper meridian’,and achieve better therapeutic effects.
