Sn100 kV在儿童副鼻窦CT检查中的应用

摘要 目的探讨Sn100 kV在儿童副鼻窦病变的应用,保证诊断效果的同时减少辐射剂量,提高儿童诊断安全性。方法回顾性分析山西省儿童医院2021年12月至2022年2月100例接受CT检查的副鼻窦病变患儿,采用Sn100 kV扫描、常规CT扫描各50例,比较2组图像质量、辐射剂量。2名医师采用Likert 5级量表对图像质量评分,计算其一致性。结果Sn100 kV组容积CT剂量指数(CTDIvol)为0.52 mGy,剂量-长度乘积(DLP)为(7.4±0.6)mGy·cm,有效直径(ED)为(0.03±0.01)mSv,体型特异性剂量评估(SSDE)为(0.55±0.02)mGy,低于常规CT组(2.6±0.4)mGy,(32.9±7.8)mGy·cm,(0.13±0.04)mSv,(2.68±0.26)mGy,分别降低了79.8%,77.5%,76.9%,79.5%,差异有统计学意义(P<0.001)。骨窗、软窗眶内脂肪和玻璃体信号与干扰加噪声比(SNR)差异均有统计学意义(P<0.001),但仍满足诊断要求。2名医师间图像质量一致性为83.8%。结论Sn100 kV扫描在不影响临床诊断的情况下,可以有效降低辐射剂量。 Objective To evaluate the diagnostic image quality and radiation dose of computed tomography(CT)using a tin-filtered 100 kV(Sn100 kV)protocol in pediatric paranasal sinus.Methods We retrospectively evaluated 100 pediatric patients who underwent CT at Shanxi Children′s Hospital of the paranasal sinus from Dec 2021 to Feb 2022 with different protocols(Sn100 kV,tube current 150 mAs,Group A,n=50;Care kV,Care dose 4D,Group B,n=50).The CTDIvol,DLP,ED,and SSDE were estimated.Noise was determined for the calculation of SNR.Two readers independently assessed subjective image quality using a five-point Likert scale.Inter-observer agreement was calculated and expressed as a percentage of agreement.Results All examinations showed diagnostic image quality regarding the evaluation of sinus disease.The CTDIvol,DLP,ED,and SSDE values were 0.52 mGy,7.4±0.6 mGy·cm and 0.03±0.01 mSv,and 0.55±0.02 mGy,respectively,in Group A,and 2.6±0.4 mGy,32.9±7.8 mGy·cm,0.13±0.04 mSv,2.68±0.26 mGy,respectively,in Group B.Compared with Group B,in Group A,the CTDIvol,DLP,ED,and SSDE were lower by 79.8%,77.5%,76.9%,and 79.5%,respectively(P<0.001).And the group A had a lower SNR(P<0.001).The agreement percentage of general image quality among observers was 83.8%.Conclusion The Sn100 kV technique enabled a significant dose reduction in CT examinations performed in the pediatric paranasal sinus while maintaining diagnostic image quality with clinically acceptable image noise.
