摘要
【目的】采用Meta分析评价真武汤加减治疗冠心病心力衰竭的临床疗效。【方法】检索中国知网(CNKI)、维普中文科技期刊数据库(VIP)、万方数据(Wanfang)、中国生物医学文献数据库(CBM)、Corchrane Libaray、PubMed等数据库中有关真武汤加减联合常规西医疗法(试验组)对比常规西医疗法(对照组)治疗冠心病心力衰竭的随机对照研究。采用Cochrane系统评价员手册推荐的质量评价工具对纳入文献进行质量评价;采用Review Manager 5.4软件对纳入文献进行Meta分析。【结果】共纳入符合要求的随机对照研究文献13篇。Meta分析结果显示,试验组的心功能改善疗效优于对照组[OR=4.46,95%CI(2.90,6.87),P<0.00001],且试验组在改善心室重构指标左室射血分数[SMD=1.47,95%CI(1.04,1.91),P<0.00001]、左室舒张末期内径[MD=-3.80,95%CI(-4.76,-2.84),P<0.00001]、氨基末端脑钠肽前体[MD=-599.16,95%CI(-633.13,-565.18),P<0.00001]方面优于对照组。【结论】在常规西药治疗基础上加减运用真武汤治疗冠心病心力衰竭,其心功能改善疗效较显著,且可有效改善冠心病心力衰竭患者的心室重构指标。
Objective To evaluate the clinical efficacy of modified Zhenwu Decoction in the treatment of heart failure in coronary heart disease by using meta-analysis.Methods The randomized controlled trials of modified Zhenwu Decoction combined with conventional western medical therapy(trial group)versus conventional western medical therapy(control group)for the treatment of heart failure in coronary heart disease were screened from the databases of CNKI,VIP,Wanfang Data,CBM,Cochrane Libaray,and PubMed.The quality assessment tool recommended by the Cochrane Reviewer's Handbook was used to assess the quality of the included literature,and meta-analysis was performed using Review Manager 5.4 software.Results A total of 13 randomized controlled trials were included.The results of meta-analysis showed that the trial group had better efficacy on improving cardiac function than the control group[OR=4.46,95%CI(2.90,6.87),P<0.00001]and had stronger effects on improving ventricular remodeling indexes of left ventricular ejection fraction[SMD=1.47,95%CI(1.04,1.91),P<0.00001],left ventricular end-diastolic diameter[MD=-3.80,95%CI(-4.76,-2.84),P<0.00001],and N-terminal pro-brain natriuretic peptide[MD=-599.16,95%CI(-633.13,-565.18),P<0.00001]than the control group.Conclusion The application of modified Zhenwu Decoction on the basis of conventional western medicine treatment for heart failure in coronary heart disease has a significant efficacy on improving cardiac function and can effectively improve the ventricular remodeling indexes in patients with heart failure in coronary heart disease.
