摘要
目的:探讨磁共振成像(MRI)诊断子宫内膜癌患者肌层浸润和盆腹腔淋巴结转移的价值。方法:选取2018年1月—2021年12月重庆市大渡口区人民医院收治的子宫内膜癌手术患者64例作为研究对象,患者均开展术前MRI检查,以术后病理检查结果作为“金标准”,分析MRI检查对子宫内膜癌肌层浸润以及盆腹腔淋巴结转移的准确性、灵敏度、特异度。结果:MRI检查对子宫内膜癌肌层浸润程度的诊断准确性为90.63%(58/64),低于术后病理检查结果,差异有统计学意义(P=0.014);MRI灵敏度为92.45%(49/53),特异度为91.43%(32/35),与术后病理检查结果比较,差异无统计学意义(P>0.05)。MRI检查对子宫内膜癌盆腹腔淋巴结转移的诊断准确性为82.81%(53/64),灵敏度为76.47%(13/17),特异度为85.11%(40/47),低于术后病理检查结果,差异有统计学意义(P<0.05)。结论:MRI对子宫内膜癌肌层浸润深度的判定具有较高的临床价值,但在盆腹腔淋巴结转移诊断中的准确性较低,故在实际诊断时,应与其他诊断方法联合应用,提高诊断准确性。
Objective:To explore the value of magnetic resonance imaging(MRI)in the diagnosis of myometrial invasion and pelvic-celiac lymph node metastasis in patients with endometrial carcinoma.Methods:A total of 64 patients with endometrial cancer surgery in the Chongqing Dadukou District People's Hospital from January 2018 to December 2021 were selected as the study subjects.All patients underwent preoperative MRI,and postoperative pathological examination was used as the gold standard to analyze the accuracy,sensitivity,and specificity of MRI for myometrial invasion and pelvic-celiac lymph node metastasis of endometrial carcinoma.Results:The accuracy of MRI in diagnosing the degree of myometrial invasion of endometrial cancer was 90.63%(58/64),which was lower than postoperative pathological examination results,and the difference was statistically significant(P=0.014);the sensitivity of MRI was 92.45%(49/53)and the specificity was 91.43%(32/35),there was no statistically significant difference in comparison with postoperative pathological examination results(P>0.05).The accuracy,sensitivity,and specificity of MRI in diagnosing pelric-celiac lymph node metastasis of endometrial carcinoma were 82.81%(5364)76.47%(1317)and 85.11%(40/47),which were lower than postoperatire pathological examination results,and the diference was satistically signifecant(P<0.05).Conelusion:MRI has high elinical value in determining the depth of myometril inasion of endonetrial carcinoma,but its accumacy in the diagnosis of pelric--celiae lyomph node metastasis is low,so it should be used in combination with other diagnostic methods to improve the diapnostic accuracy in actual diagnosis.
