摘要
[目的]分析疏肝调脾化瘀汤联合经肝动脉化疗栓塞术(TACE)治疗肝郁脾虚型原发性肝癌的临床疗效及对患者肝功能与肿瘤相关指标的影响。[方法]在广西中医药大学第一附属医院的住院信息管理系统(HIS系统)中收集确诊为肝郁脾虚型原发性肝癌且均行TACE治疗的92例患者作为研究对象,根据有无服用疏肝调脾化瘀汤分成观察组和对照组各46例。对照组患者予TACE治疗以及常规的抗病毒、保护肝功能、降低转氨酶等治疗,观察组在对照组治疗的基础上给予疏肝调脾化瘀汤治疗。比较两组治疗前后的肝功能指标以及肿瘤相关指标的变化情况。观察指标包括丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、谷氨酰转移酶(GGT)、总胆红素(TBIL)、白蛋白(ALB)、凝血酶原时间(PT)、甲胎蛋白(AFP)及相关肿瘤指标[糖类抗原125(CA125)、糖类抗原153(CA153)、糖类抗原199(CA199)]等。[结果]观察组总有效率为60.87%,对照组总有效率为34.78%,两组疗效差异有统计学意义(P<0.05)。治疗后观察组患者ALT、AST、GGT、TBIL、ALB、PT、AFP、CA125、CA153及CA199指标均较对照组有明显的改善(P<0.05)。[结论]疏肝调脾化瘀汤联合TACE治疗有利于改善肝郁脾虚型肝癌患者的肝功能、凝血功能以及降低相关肿瘤标志物水平,进一步增强治疗效果,具有临床应用价值。
[Objective]To analyze the clinical efficacy of Shugan Tiaopi Huayu decoction with TACE in the treat⁃ment of primary liver cancer with liver depression and spleen deficiency and its effects on liver function and tumor-re⁃lated index levels.[Methods]Ninty-two patients diagnosed with hepatocellular carcinoma of liver-depression and spleen-deficiency type were collected from the first affiliated hospital of guangxi university of chinese medicine by hos⁃pital information system(HIS)as study subjects.They were divided into observation group and control group,46 cases in each group.The patients in the control group were treated with TACE for liver cancer and conventional antiviral,liv⁃er function protection,and transaminase reduction.The observation group was treated with Shugan Tiaopi Huayu decoc⁃tion on the basis of the control group.The changes of liver function indexes and tumor related indexes before and after treatment were compared between the two groups.The observation indexes included alanine aminotransferase(ALT),aspartate aminotransferase(AST),glutamyl transferase(GGT),total bilirubin(TBIL),albumin(ALB),prothrombin time(PT),alpha fetoprotein(AFP)and related tumor indexes[carbohydrate antigen 125(CA125),carbohydrate antigen 153(CA153),carbohydrate antigen 199(CA199)].[Results]The total effective rate was 60.87%in the observation group and 34.78%in the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,ALT,AST,GGT,TBIL,ALB,PT,AFP,CA125,CA153 and CA199 in the observation group were significant⁃ly improved compared with those in the control group(P<0.05).[Conclusion]Shugan Tiaopi Huayu decoction com⁃bined with TACE is beneficial to improve liver function,coagulation function and reduce the levels of related tumor markers in patients with liver cancer of liver depression and spleen deficiency,and further enhance the therapeutic ef⁃fect,which has clinical application value.
